I. Rating System

exceeds requirements.
4: 88 - 95 Performance often excessed requirements Very Satisfactory (VS)
3: 81 - 87 Performance meets requirements Satisfactory (S)
2: 75 - 80 Performance needs more improvement Fair (F)
1: 70 -74 Performance is below expected level Needs Improvement (NI)

II. Criterions: (Please put only 1 mark that suit into the interns performance).

A. Punctuality and Attendance (10 pts) Put a Mark

B. Knowledge of Work (15 pts)

C. Quality of Works (10 pts)


(10) Complete and accurate in all details
(8) Rarely commits error
(6) Fairly good and seldom commits error
(4) Requires occasional practice
(2) Very careless and often time commits error

D. Acceptance of Responsibilities (10 pts)


(10) Consistently acts in a responsible manner
(8) Shows marked interest in his/her work
(6) Display normal interest in his/her work
(4) Has a little interest in his/her work
(2) Manifest a very low regard for his/her work

E. Acceptance of Direction (20 pts)


(20) Carry out direction to the best of his/her ability
(16) Shows keen interest in accepting direction
(12) Accepts direction and carries out with minimum correction
(8) Needs constantly prodding and display passive attitude
(4) Shows inability to carry out direction.

F. Personal Appearance and Bearing (15 pts)


(15) Always clean, neat, well-groomed and always in prescribe attire
(14) Rarely seen in proper attire, clean and well-groomed

(8) Hardly observes good grooming
(4) Needs more improvements on his/her appearance

G. Moral Values and Ethical Standards (20 pts)
