Omta Merit Certificates: Guidelines For Ordering (See Page 2 For Fillable Order Form)

OMTA merit certificates are awarded to students who complete a minimum of 30 lessons in an academic year with their OMTA instructor. The certificates come in different styles depending on the year of study completed, from 1 to 14, and are ordered by instructors to give to students in May or June. Instructors provide a student list and payment to the OMTA office at least 3 weeks before needed so that certificates can be printed with student names and presented.

OMTA MERIT CERTIFICATES

OMTA merit certificates are available to OMTA instructors and are earned by students who have
completed a minimum of 30 lessons during the current teaching year.

These attractive, parchment-like half-sheet certificates (levels 10-14 are full-sheet) are stamped with
the OMTA embossed seal. Years 2 through 14 are further enhanced by gold seals and colored
ribbons. A year represents a year of study, 30 lessons or more, and is not related to Syllabus levels.
The determination of the year level of each student’s award and all record keeping are the teacher’s
responsibility.

An example of the certificate can be viewed below. The student’s name, year level, and the teaching
season (e.g. 2014-2015) will be printed on the certificate. Certificates are stamped with the signature
of presiding OMTA President, and there is a space for the teacher to sign. They are typically
awarded in May or June and should be ordered three weeks in advance of the time the teacher
wants to give them to students. You may pre-order via e-mail, office@oregonmta.org.

Guidelines for Ordering (see page 2 for fillable order form):

3. Include a check or money order payable to OMTA ($.75 per certificate).
