FMEA Business Process Example

This FMEA form is used to analyze potential failures in a process or product. It lists two potential failure modes for an invoicing process: incorrect amount and incorrect address. The effects of an incorrect amount are reduced customer satisfaction and late payment of invoices. The effects of an incorrect address are that the customer does not receive the invoice, resulting in lower satisfaction and late payment. Potential causes and current controls are identified for each failure mode. Recommended actions are provided to reduce the risk priority number assigned to each failure mode.
