This document is an affidavit of denial signed by eight daughters of the late Lucena Valdez. They state that 1) Lucena S. Valdez was born on a specific date in a specific location, 2) there is a record of a Lucena Valdez with a loan at Landbank of the Philippines but 3) the Lucena Valdez in the bank record is not the same person as their mother. The daughters signed the affidavit to deny that their mother is the same person in the bank record.

Brgy. Camanacsacan, San Jose City, Nueva Ecija, VIOLETA L. DAYOS - (daughter) of legal
age, and a resident of Brgy. Camanacsacan, San Jose City, Nueva Ecija, ESTRELLA L.
NITURA - (daughter) of legal age, and a resident of Brgy. Camanacsacan, San Jose City, Nueva
Ecija, PERLITA L. TABLADA - (daughter) of legal age, and a resident of Brgy. Tabulac, San
Jose City, Nueva Ecija, FELICIANA L. BALDOVINO - (daughter) of legal age, and a
resident of Brgy. Tabulac, San Jose City, Nueva Ecija, INOCENCIA VALDEZ LLENA -
(daughter) of legal age, and a resident of Brgy. Camanacsacan, San Jose City, Nueva Ecija,
INOCENCIA VALDEZ LLENA - (daughter) of legal age, and a resident of Brgy.
Camanacsacan, San Jose City, Nueva Ecija, CALIXTA L. NAVA - (daughter) of legal age, and
a resident of Brgy. Camanacsacan, San Jose City, Nueva Ecija, the heirs of late LUCENA
VALDEZ, after having been duly sworn to in accordance with law, hereby depose and state
THAT:

1. That the true and lawful name of our mother is LUCENA S. VALDEZ, born on
October 21, 1990 at Carranglan, Nueva Ecija;

2. There has been LUCENA VALDEZ according to the records of Landbank of the
Philippines, South Nueva Ecija Accounting Center-Area IIIB, Cabanatuan City
during the processing of her Statement of Account Summary her total loan was
FOUR THOUSAND THREE HUNDRED THREE AND 73/100 (PHP.4,303.73.00) ;

3. That we executed the foregoing to attest to the fact that she’s not the same LUCENA
VALDEZ as recorded in the Landbank of the Philippines, South Nueva Ecija
Accounting Center-Area IIIB, Cabanatuan City;

4. And so, we executing this affidavit for whatever legal purpose or purposes this would
serve to attest the facts as stated above.

AFFIANT SAYETH NONE FURTHER.

IN WITNESS WHEREOF, I have hereunto affixed our signature this _ day of July 2017,
here at Science City of Muñoz, Nueva Ecija.
