AFFIDAVIT OF LOSS
I, CELESTINO PIZARRO ACOSTA, legal age, married, and a
resident of Davao City, after being sworn duly sworn to in accordance with
law do hereby depose and say that:
1. I am a holder of a Philippine National Bank ATM Card as a
savings account depositor thereof;
2. The ATM Card was in my possession from the time of its issuance
but on or about the third week of October 2014 when I
inadvertently lost it when I last withdrew money on such date;
3. Notwithstanding diligent efforts exerted to find it; the same was
not recovered, hence, considered lost or destroyed for all its
intents and purpose;
4. I am executing this affidavit in order to attest to the truth of
foregoing circumstances and for the purpose of informing
Philippine National Bank of such fact of loss and for securing a
replacement thereto as well as for any other legal purposes this
affidavit may server.
IN WITNESS WHEREOF, I have hereunto set my hand this ___ day
