all claims I may have against my employer and its agent by reason of my recruitment.

In consideration of my receipts of the above sum.

I acknowledge the value of the opportunity afforded me to be of service to the Company:

I release, remiss and forever discharge said Company, its stockholder, its officers, directors and
employees from any actions, sum of money or other obligations arising from all incidents of my
employment with the Company;

I acknowledge that I have received all amounts that are now or in the future may be due to me
from the Company;

I will not at anytime, in any manner whatsoever directly or indirectly engage in any activity
prejudicial to the interest of the Company, its stockholders, its officers, directors and employees and will
not disclose informations concerning the business of the Company, its manner of operation, its plans,
processes or data of any kind;

I Further acknowledge that I have no cause of action, complaints, case or grievances whatsoever
against the Company, its stockholders, its directors and officers in respect of any manner incidents to or
arising out of my previous employment with the Company or the cessation / termination of my
employment. Moreover, I further warrant that I will institute no action and will not continue to
prosecute pending actions, if any, against the Company; its stockholders, its officers, its directors/
employees. I also manifest that the acceptance of payment to me of the above sum by the Company
shall not be taken by me, my heirs or assigns as a confession or admission of liability on the part of the
Company, its stockholder, its officers, directors employees for any matters, cause, demand or damages I
may have against any or all of them.

I finally expressly declare that I have read these documents and the release, waiver and
quitclaim hereby given are made knowingly, willingly and voluntarily and with full knowledge of my
rights under the law.

In witness whereof, I have hereunto set my hand this ______ day of _____________ at San Juan,
