JOINT AFFIDAVIT

We, Pcpl Love Hernandez and Pcpl Shine L. Diamond,


both of legal age, married, Filipino citizens and residents of
Bocaue, Bulacan, Philippines and after having been duly
sworn to in accordance with law hereby depose and say that;

1. We are a bonafide member of the Philippine National Police


(PNP for brevity) and presently assigned at Bocaue City
Police Office 3 (BCPO3) with office address at Barangay
Tabi, Bocaue, Bulacan, detailed at Women and Children
Protection Desk (WCPD for brevity) as duty Investigator
under office order no. 45678 dated 01 April 2018;

2. Sometime on 29 December 2019, on or about 8:30 in the


morning, we were at the WCPD, when the mother of the
complainant/victim identified as NANCY ADOLFO, 48
years old, a resident of Barangay Poblacion, Bocaue,
Bulacan, appeared and reported to our office that her
daughter RONNA ADOLFO (RONNA for brevity) was
allegedly raped by an unknown perpetrator;

3. Thus, on or about 10:00 in the morning of the same date


after the initial interview with Ronna, we immediately
proceeded at Barangay Poblacion, to conduct ocular
investigation where the rape incident transpired involving
the abovementioned complainant/victim RONNA;

4. Upon arrival thereat, we cordoned said area by using


crime-scene tape to preserve the crime scene and/or to
protect/prevent the evidence from being contaminated.
Secured the pieces of evidence in a clean air tight plastic
container such as :

a. Bloodstained blanket and bed sheet/ cover;


b. An empty bottle of San Miguel Beer with some blood
residue on the tip and droplets of blood inside the
bottle at the bottom;

5. Thereafter, we took photographs of the scene, took several


overview images of the surrounding areas from multiple
angles and point of view so that we can show the scene
preserved in an unaltered condition;

6. Pieces of evidence was immediately transferred to the


crime laboratory for the conduct of appropriate
examination;

7. We execute this affidavit to attest to the truth of the


foregoing facts based on our personal knowledge and
authentic records that we are the Police Investigators who
conducted an ocular investigation in the said location and
to prove the veracity of our statement.

IN WITNESS WHEREOF, We have hereunto set our hand


on this 3rd day of January, 2020 at City of Malolos, Bulacan,
Philippines.
