What is Ideal Dosage Regimen?
• An ideal dosage regimen in the drug therapy of any
disease is the one which immediately attains the
desired therapeutic concentration in plasma and
maintains it constant for the entire duration of
treatment.
• It is achieved through administering a drug in a
particular dose at a particular frequency.
• The dosing interval of any drug depends upon

• A conventional formulation/ Immediate release (IR) dosage
form is one which is designed to release the drug contained
in it rapidly and completely after the administration.
Here the dosing interval is much shorter than the half life
of the drug resulting in several limitations
Limitations of Conventional dosage forms
 Poor patient compliance, which increased
chances of missing the dose of a drug with short half
life for which frequent administration is necessary.
 The unavoidable fluctuations of drug
concentration may lead to under medication or
over medication.
 A typical peak-valley plasma concentration time
profile is obtained which makes attainment of steady
state condition difficult.
 The fluctuations in drug levels may lead to
precipitation of adverse effects especially of a
drug with small Therapeutic Index (TI)
whenever over medication occur.

An ideal drug delivery system should deliver the drug at
desired rate over a specified period of treatment. The two

Extended Release [ER, XR, XL] : Reduction in dosing frequency


through prolonging the drug release profile

Delayed Release [DR]: Release the drug at a time other than


promptly after administration

Targeted Release : Release the drug at a specific target/site in the


body
Sustained Release [SR] : Provide an immediate dose for
normal therapeutic response, followed by maintenance dose
to sustain the therapeutic response for specific extended
period of time usually 8-12 hrs

Controlled Release [SR] : Release the drug at a


predetermined constant rate, for a specific period of time,
locally or systematically
