AFFIDAVIT OF OWNERSHIP, LANDHOLDING AND NON-TENANCY

AND/OR (LEASEHOLD)

KNOW ALL MEN BY THESE PRESENT;

That I, ______________________________________, of legal age, single/married,


Filipino citizen and with postal address at _____________________________________,
after having duly sworn to in accordance with the law depose and say;

That I am the lawful owner of that certain parcel of land described as Lot No.
_____________, Csd- _________________, identical to Lot No. _________________,
Cad. ___________, _________________________ Cadastre, with an aggregated area of
_______________________square meters and located at Barangay ________________,
________________________Laguna.

That the above-cited parcel of land has been in my possession and occupation for
more than thirty (30) years either by myself thru my predecessors-in-interest and had
developed the same into productive land and that the same is declared for taxation
purposes under Tax Declaration No. __________________________________________.

That the same is covered by Free Patent Application No. _____________, on file at
the Community Environment and Natural Resources (CENRO) at Los Baños, Laguna and
that the subject land is not tenanted neither the same is covered by a leasehold contract
with other persons;

That I hereby state that my landholding does not exceed the maximum retention
limit set forth by the Comprehensive Agrarian Reform Program (CARP) of the government;

That the same lot is not claimed by any other person and that I hold myself solely
responsible should the land subject thereof is involved in any case whether civil or
criminal;

That I executed this affidavit in compliance with the instruction of the Provincial
Environment and Natural Resources Officer (PENRO), Los Baños, Laguna for the purpose
of release of the Free Patent to the land pursuant to the provision of Republic Act No.
9176;

That I am not a CARP beneficiary and have not had the opportunity to obtain such
capacity form the DAR.

IN WITNESS WHEREFORE, I have the hereunto set my signature in this


instrument after the same has been read and understood by me this _______ day of
