This memorandum of agreement allows students and advisers from the National Service Training Program of Dalubahasaan ng Lungsod ng Lucena to adopt a purok (small community) as the area for their civic welfare training program. The barangay agrees to the adoption and outlines requirements for the students, such as abiding by barangay rules and focusing projects on values, health, literacy and more. MEMORANDOM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:


This contract affiliation is made and executed this day of 20 in the City of Lucena
by and between: Barangay Lucena City, with the
office address at Barangay Lucena City, represented
in the act by Punong Barangay, herein referred to as
the PARTY OF THE FIRST PART:
And
The National Service Training Program of Dalubahasaan ng Lungsod ng Lucena, with office
address at Barangay Isabang, Lucena City represented in this act by MR. RONALD C. MACINAS
Coordinator community affairs, herein referred to as the Party OF THE SECOND PART:

WITNESSETH
1. That the PARTY OF THE FIRST PART hereby designates the PARTY OF THE FIRST PART
to accept such designation under the terms and condition hereinafter set forth;
2. That the PARTY OF THE SECOND PART shall allow the CIVIC WELFARE TRAINING
SERVICE-NATIONAL SERVICE TRAINING PROGRAM DALUBHASAAN NG LUNGSOD NG
LUCENA student and adviser of the SECOND PART to adopt the PUROK
as the community/area for CIVIC WELFARE TRAINING PROGRAM
of the DALUBHASAAN NG LUNGSOD NF LUCENA under of the following requirements.
a) The adviser and student of the PARTY OF THE SECOND PART shall abide by the
rules and regulation of the PARTY OF THE FIRST PART.
b) The projects/programs of the PARTY OF THE SECOND PART will covers the
following areas: Moral/Values reformation, Health, Literacy, Environmental
Protection, Livelihood, Food and Nutrition, and Socio-Cultural activities.
c) Any infrastructures that the PARTY OF THE SECOND PART plan to construct will
be first consulted to the PARTY OF THE SECOND PART.
d) Adviser and student will strictly adhere professional decorum during the
community activity.
e) Discipline will be imposed on the student by the adviser/coordinator to ensure
safety and avoidance of any unprecedented events.
f) Adviser and student of the PARTY OF THE SECOND PART not be whatsoever
collected with unsolicited amount for any unnecessary purposes by the PART OF
THE FIRST PART.
3. Any violation of the above provision shall continue sufficient ground for the immediate
cancelation of this contract.
IN WITNESS WHEREOF the parties hereunto set their hands on day of 20
in Lucena City, Philippines.
