duly sworn in accordance with law, hereby depose and state:

1. That I, together with my co-depositors, were issued Certificate

2. That recently, however, I tried to look for the said Certificate of

Certificate of Time Deposit, I could not find the same such that I now
believe that it is now lost beyond recovery;

4. I am executing this Affidavit of Loss to attest to the truth of the


foregoing and for the issuance of a new Certificate of Time Deposit in lieu
of the one that was lost, and for any legal purpose that this may serve.
