Proposed Job and Isolation description that is outwith Recommended Isolation Selection Chart

On “B” Recycle valve on WREP, FV-61-NO2AO2 A,B,C the valve plugs are to be changed out for
new design. Valve body to be left in place, bonnet removed & plug changed. Suction valve
XV 61NO2AO1 ABC and discharge XV 612NO2A02A02 ABC to be hand jacked shut, power
removed & vent valves open.

Precautions to be taken in support of proposed Isolation to reduce to lowest reasonably practicable


level the potential hazard

1. Valve’s hand jacked shut, Actuator power removed ( Suction & Discharge).
2. Vent at suction pressure gauge to be open.
3. Line’s & pump drained
4. Operator to stand by to check for leakage

Reason why Recommended Isolation Selection Chart is not being applied

Isolation requirement is for single isolation & blind, time and risk exposure for installation of blinds
is estimated to be 4 hrs. Requires opening pipe work numerous times. Recycle line is high point of
system, Crude is stabilized, associated gas is minimal.

Consequences of the proposed Isolation Failing

Oil Leaking to concrete pad.


All valves are ball valves, leakage probability is minimal.
