Trichomoniasis and Candidiasis

Trichomoniasis and candidiasis are common causes of vaginitis. Trichomoniasis is caused by the protozoan Trichomonas vaginalis, which is predominantly transmitted sexually. It causes inflammation and irritation in the vagina. Candidiasis is caused by the yeast Candida albicans, and predisposing factors include diabetes, pregnancy, antibiotics, oral contraceptives, and immunosuppression. It produces a thick, white vaginal discharge and intense itching. Homeopathic remedies for vaginitis include Thuja and Apis mellifica.

TRICHOMONIASIS AND CANDIDIASIS

Trichomonas Vaginitis :
Vaginal Trichomoniasis is the most common & important cause of vaginitis
in the child learning period.

Causative Organism :
 It is caused by Trichomonas vaginalis , a pear shaped unicellular flaguate
protozoa .
 It measures about 20 M long and 10 M wide.
 It has got four anterior flagellae & a spear like profussion at the other end
with an undulating membrane surrounding its Ant 2/3.

Mode of Transmission :
 The organism is predomonantly transmitted by sexual contact, the male
harbours the infection in the urethra and prostrate.
 Transmission : Toilet articles from one woman to the other or through
examining glones.
 The incubation period is 3-28 days.

Pathology :
 Women in reproductive period , the parasites harbour in the vagina in
asymptomatic stat.
 When the local defense is impaired during and after menstruation, after
sexual stimulation & following illness.
 The PH of the vagina is raised to 5.5 – 6.5. At this level of PH trichomonas
thecine.
 The organisms usually lie in between the rugae & produce surface
inflammatory reaction. when the defense is lost.

Clinical Features :
a. There is sudden profuse & offensive vaginal discharge often dating from last
menstruation .
b. Irritation & itching of varrying degress within and around the introitus are
common.
c. There is presence of urinary symptoms such as dysmnia & frequency of
mictuition.
d. There may be history of prenious similar attacks.

On Examination :
a. There is thin, greenish yellow & frothy offensive discharge per vaginam.
b. The vulva is inflamed with evidence of purelent smell & pruritius.
c. Vaginal examination may be painful. The vaginal wall becomes red and
inflamed with multiple punctate slaemorrhagic spots.
d. Similar spots are also found over the vagina mucus of the portio vaginalis.
e. Giving appearance of strawberry.

Diagnosis :
a. Identification of Trichomonads is done by hanging drop preparation
b. Confirmation may be done by culture.
CANDIDA VAINITIS
(Moniliasis)
Causative Organism :
Moniliasis is caused by candida albicans, a gram positive yeast like fungas.

Predisposing factors for candida vaginitis :


Diabetes : Increase Glycogen in the cells, glycosuria
Pregnancy : Increase vaginal acidity, glycosuria
Broad Spectrum antiblotics : decrease Acid forming lactobacillus
Combined Oral Pills :
Immuno Suppression : HIV
Drugs : Steroids
Thyroid: Parathyroid Disease

Clinical Features :
The patient complains of vaginal discharge with intense vulvovaginal preritus.
The preritus is out of proportion to the discharge.
There may be dysparenia due to local soreness.

On Examination :
a. The discharge is thick curdy white & in flakes. often adherent to the vaginal
wall.
b. Vulva may be red and swollen with evidences of pruritus.
c. Vaginal examination may be under removal of white flakes reneals multiple
oozing spots.
Homoeopathic Remedies For Vaginitis

1. Thuja :
 Vagina is very sensitive , warty growth on vulva and perineum.
 Patient has warts condyzomata on external genitalia.
 History of suppression og gonorrhoea.
 Profuse lucorrhoea which is thick and greenish.
 Infection sets in after the over growth & which then ascends to fallopian
tubes & ovaupes.
 Sense pain in left ovary & in left inguinal region.
 Menses are scanty & retarded.
 Dryness of wall of vagina.
 Coition very painful.
 Thuja has antibacterial action.
 < Damp met urether, in the evening washing, after laundry work.
 > Hard pressure, warmth, cold bathing, after sleep.

2. Apis Mellifica :
 Acts on vaginal tissues, which are oedematous and puffy due to diseased
condition.
 There is swelling of vulva, labia with stinging type of pains.
 Soreness with intolerance of heat.
 Touch causes intense pain.
 There is sense of tightness & bearing down as if menses are suppressed.
 Inflammation of the endometrial lining of the uterus
 Cystic swelling of ovary gets infected.
 < sleep, hot things, touch, bandaging, clothings
 > cold application, open air.
