1.0 INTRODUCTION
The intention of this method statement is to provide with a basic understanding of
the construction methods and associated safety procedures to be adopted on this
project. This document is intended as a working document and as a result will be
subject to amendments where products, materials or methods change to improve
the quality of our product and the safety of our work site. Amendments may be
made to improve the efficiency or practicality of the producers provided that safety is
not compromised.
2.0 REFERENCE
A work activity specific hazard identification and risk assessment has been carried out
for this work activity (see attached risk assessment), with suitable control measures
indicated. The control measures as contained in the approved risk assessment must be
followed as applicable.
3.2 SAFE WORK PRACTICE
Work shall not commence until all necessary permits have been obtained.
Briefing/STARRT meeting shall be conducted by Supervisor and Safety officer before
initiating the activity.
All necessary personnel protection equipment, such as safety shoes, overall,
safety goggles, helmet, safety harness, etc. will be provided and worn.
Workers shall be competent and work at height trained.
Approved lifting plan, as prepared by a competent Lifting Supervisor, shall be followed.
All lifting personnel shall be trained and certified by recognized third party certifying bodies.
All lifting gears and equipment shall be certified by recognized third party certifying bodies.
Suitable fall arrest system, with secure anchorage points, shall be provided to all workers
exposed to fall from height hazard.
All power tools must be properly colour coded.

Full body harness and lanyards must be visually inspected, daily before use.
Good housekeeping practice shall be maintained in work area.
Safety officer and supervisor shall supervise work activities.
1. Safety officer shall issue appropriate work permits for the activity.
2. Lifting supervisor shall obtain Lifting permit.
3. STARRT meeting shall be conducted for workers and attendance recorded.
4. Exclusion zone shall be maintained on lower areas of the work zone using hard
barricades and adequate signages. Unauthorized entry into these exclusion zones shall
be prohibited.
5. All lifting activities shall be carried out by personnel trained to carry out lifting activities
(Lifting personnel).
6. Good communication shall be maintained between lifting team and affected workers
(panel installers).
7. Installers shall don full body harness with lanyard and anchor to suitable anchorage
points provided.
8. Installers, and any employee that performs any activity at the edge of floor, shall use chin
straps to prevent their helmets from falling down from height.
9. Installers shall use tool tethers to attach power and/or hand tools to their person to
prevent them from dropping to lower level.
10. Permit shall be obtained for edge protection removal before edge protection can be
removed.
11. The edge protection shall be removed sequentially – only the edge protection in the
immediate work area shall be removed and it shall be re-installed on completion of works
in that area before another edge protection is removed.
12. Housekeeping shall be done on as-you-go basis. Waste shall not be left to accumulate in
the work area.
13. All power tools shall be disconnected from power source and kept away from walkways
when workers are going on break and at end of shifts.
14. All permits shall be closed by the permit requester and handed in to the Safety officer at
the end of each shift.
The emergency procedure depends on the type of emergency, and each shall be as outlined
below:
3.4.1 FALL AND RESCUE FROM HEIGHT
The method of rescue to be adopted shall be the CRANE MAN BASKET RESCUE as
contained in the approved Working At Height Rescue Plan of SKYWALL.
The summary of the sequence of steps to take is shown in the table below:

Perform situation assessment and coordinate with OE to get the Crane and Man basket ready
for Man Basket Rescue as soon as possible.
Ensure there are sufficient first aid kits and lanyards in the rescue man basket.
Provide, or assist the first aider while providing, first aid to the fallen worker.
The crane operator must be trained to perform crane operations using a Man Basket
to carry workers.
The company must ensure that the Crane, the Man Basket and the fall protection
required is in accordance with QCS 2014.

The Rescue team shall review this procedure at least every two (2) weeks with the
Crane (Lifting) crew.
At least one rescue team member will get onto the man basket and make sure there
is a second shock absorbing lanyard for the fallen worker who is being rescued.
The Crane will be maneuvered into position and then raised up under the worker to
be rescued in the same manner as the mock drill.
The rescue team member will attach the second lanyard from the Man Basket to the
fallen worker to be rescued.
Before releasing the lanyard or self-retracting life (SRL) line that arrested the
worker’s fall, the Rescue team leader, one other member of the rescue team and the
fall victim (if capable) must all verify a positive connection from the personnel
platform anchorage point to the fall victim’s harness.
The Rescue team member, after receiving permission from the Rescue team leader
may disconnect the lanyard or SRL involved in arresting the worker’s fall.
Lower the Man basket to the ground with the rescued worker.
The rescued worker must receive prompt medical attention for all serious injuries,
including treating for possible trauma.
Safety of Rescuers:
1. The rescue team members shall be provided adequate training by known and
approved third party training and certification body.
2. Rescue training records of rescue team members must always be up to date.
3. The number of rescue team members must be sufficient to handle foreseeable
emergency rescue needs. This is to ensure rescuers are not overwhelmed
during rescue operations.
4. At least two members of the rescue team must be certified First Aiders.
5. Rescue equipment selected must appropriate for the nature of work.
6. Any obstructions are in the way of reaching the suspended employee must be
assessed and a plan to avert it should be thoroughly considered.
7. The actual plan to rescue the suspended employee shall be Crane with Man
basket.
8. SKYWALL shall coordinate with OE for mock drill for preparation and awareness
of SKYWALL rescue team.

Upon being notified about the fire emergency, each worker must:
Turn off and disconnect tools and equipment unless doing so jeopardizes his
safety
Leave the work area using the designated escape routes.
Report to the nearest ASSEMBLY POINT.
Remain outside until the competent authority (Safety personnel) announces that
it is safe to re-enter.

Perform situation assessment and contact OE for help in case there is a need to
stop equipment and vehicles belonging to other companies safely or the situation
is beyond what SKYWALL can handle alone.
All other emergencies, such as Environmental emergencies, Biological related emergencies etc. shall
be handled as follows:
1. The SKYWALL Safety department shall be informed immediately.
2. The SKYWALL Senior Safety Officer shall ask OE for help and guidance immediately it is
notified of any of these emergencies.
3. Where an accident results in a fatality, the scene must be cordoned off and not interfered with
until OE personnel arrive the scene and give further instructions.

Sequence of Work
The sequence of work will be as follows:
1.1 Panel Lifting and Installation by Floor Crane system or Monorail System
a.) Floor crane / Monorail hoisting system are employed on site to enable ease and speed of
installation. It consist of a counter weight with members supported on a hallow section
frame. A winch apparatus or motor is secured on to the cantilever arms and is capable of
lifting up to one ton.
b.) The slide in lifting bracket will be positioned into the top transom. This slide in bracket is held
in position by fixing bolts through pre-drilled holes in the transom fins.
The wire and shackles are then hooked onto the eye of the slid-in-bracket. The panel is now
ready to be hoisted.

h.) The panel is now dropped and engaged with the panel below and the hook
bracket in panel is engaged with the floor bracket. The panel is then slide in
to the adjoining panel mullion member. The bolts are tightened to firm the
level of panel installed.
