P l a i n t i f f J U A N T A M A D , b y counsel and unto this Honorable Court, respectfully
states that:
1. Plaintiff received a copy of the Sheriff’s Report dated 09 June 2020,
stating therein that Sheriff DE PERALTA returning t h e o r i g i n a l o f t h e
S u m m o n s f o r d e f e n d a n t J U A N S I P A G f o r t h e reason, to wit: “ P e r
i n f o r m a t i o n f r o m r e s i d e n t s o f B r g y 1 , V i g a n C i t y , defendant
JUAN SIPAG resides at Baluarte Hills, Vigan City.”
2. In view of such information, Plaintiff respectfully prays for t h e i s s u a n c e
o f a l i a s w r i t o f s u m m o n s i n o r d e r t h a t summons and a copy
o f t h e c o m p l a i n t w i l l b e s e r v e d t o Defendant JUAN SIOPAG to the
address Baluarte Hills, Vigan City
PRAYER
W H E R E F O R E , i t i s r e s p e c t f u l l y p r a y e d o f t h i s H o n o r a b l e Court to take
note of this Manifestation and Issuance of Alias Summons be granted.
