Checks patient’s chart for order for NG tube insertion. 2. Performs hand hygiene. 3. Gathers and cleanses necessary equipment and supplies. 4. Introduces self to the patient and family. 5. Identifies the patient. 6. Provides privacy. 7. Explains procedure to the patient. 8. Discusses the associated discomforts that may be experienced and possible interventions that may allay this discomfort. 9. Raises the bed to a comfortable working height. 10. Performs abdominal assessment. 11. Positions the patient with the head of the bed at least 450 or a sitting position. 12. Places the incontinent pad. 13. Has the emesis basin and tissues handy. 14. Dons non-sterile gloves 15. Measures the distance to insert the tube. 16. Marks the tube with a marker or tape. 17. Lubricates the tip of the tube (at least 2 inches to 4 inches) with water-soluble lubricant. 18. Applies topical anaesthetic to nostril and oropharynx, as appropriate. 19. Gently inserts the tube into the nostril while directing the tube upward and backward along the contour of the nose. 22. When the pharynx is reached instructs the patient to touch their chin to their chest. 23. Encourages the patient to sip water through a straw or swallow even if no fluids are permitted. 24. Advances the tube in a downward and backward direction when the patient swallows. 25. Stops when the patient breathes. 26. Checks the placement of the tube with a tongue depressor and a flashlight and stop advancing the tube if gagging and coughing persists. 27. Intervenes appropriately id met with resistance. 28. Intervenes appropriately if the patient exhibits signs of distress. 29. Stabilizes the tube while checking for appropriate placement. 30. Uses multiple methods to determine proper placement. 31. Clamps the tube. 32. Secures the tube with tape. 33. Closes the end of the tube and removes clamp. 34. Assists the patient with oral hygiene. 20. Observes the patient’s response during and after the procedure. 21. Provides appropriate instruction thought the procedure. 22. Offers comfort and reassurance throughout the procedure as necessary. 23. Removes gloves. 24. Ensures patient safety.
