COLLEGE OF NURSING

FACULTY CLINICAL GUIDELINES

Effective August 2023

I. ORIENTATION

A. Of the Faculty in the Clinical Area or Setting:

1. Pre-requisite to a faculty with clinical duties as part of his/her teaching load


should comply to the orientation of the affiliating health facility prior to the actual
duty with the students. This is to ensure the following:
a. appreciation of the facility’s policies affecting the affiliation of students
b. initial establishment of rapport with the hospital personnel including those
who are assigned in the specific area of assignment;
c. familiarization of the physical set-up of the hospital in general and the
specific unit of assignment;
d. familiarization of the nature of patients’ cases to assist in his or her student
assignment
B. Of the faculty with the assigned students or clinical group:

The conduct duty orientation can either be done a day before the actual duty or a 15-
30 minutes the actual duty with the students to discuss the following pertinent to the
duty:
1. Expectations
2. Duty uniform (varied per area)
3. Requirements (Varied per area)
4. Grading system (same but requirements per area vary)
5. Rubrics / Evaluation tools
6. Guidelines / Do’s and Don’ts
7. Forms/templates/slips as appropriate

II. GROOMING

A. Hair-cut
a. Male: Neatly cut, no fashion design (Beard & moustache NOT allowed)
b. Female: Long hair should be neatly tied in a bun; should not touch the
collar of uniform
c. Bangs should not touch eyebrows
III. UNIFORM:

IV. MAKE-UP & JEWELRIES

a. Lightface and lip make-ups are allowed


b. Wrist watches and/or weddings are the only jewelries allowed
c. Eyelash extensions are not allow
d. No nail polish, nails should not extend beyond fingertips
e. Tattoos should be covered
f. Wear white face mask (KN95 or mask with higher layers, filters,
efficiency)

V. PUNCTUALITY AND ATTENDANCES

a. Report at least 15mins before shift (BGH:1 hour before duty on day 1)
b. Make sure to Bundy in and out if Bundy machine is not working, ask the
staff/charge/head nurse to countersign the time in and out entries
c. End duty on time (especially 3-11 shift; Ensure SNs are able to take a ride
before leaving the hospital premises)
d. Inform DH at once if you cannot attend to your duty
e. As much as possible, absences should be planned to allow time to arrange
for a substitute clinical instructor;
f. In case of an emergency leave, the faculty concerned is responsible of
informing the Clinical Coordinator if not the Level/Department Head, or
the Associate Dean, as necessary. Prior arrangement with other faculty to
cover your shift is appreciated for absence not anticipated or emergency in
nature

VI. STUDENT SUPERVISION:

a. Prescribed student-patient ratio: 1:1 on first day; 1:2 depending on the


capabilities of students for the succeeding days
b. Receive end-of-shift report together with SNS;
c. Accompany students in performing assessment, drug administration,
nursing procedures especially if they have never done it before
d. Never allow SN to feed patient via NGT alone
e. Never allow SN to reconstitute vials without you supervising them
f. Never administer any medication you have not preparedg. Only 2-3
students are allowed to have their break time at any given time
g. Ensure SNs having break to endorse their patient to fellow SNs prior to
leaving the area
h. Sign every nursing skill performed or assisted by SNs in their Competency
Form. Please take note that there should be one signature per skill per
rotation only.

VII. BEHAVIORAL CONDUCT


The CI , at all times, models desirable qualities and traits of a respectable professional
nurse.
a. Do not discuss cases of patients along hallways, elevators, corridors,
public transportation
b. Do not report to duty under the influence of alcohol and/or drugs
c. Spend time establishing rapport with staff nurses, but do not consume
much of their time
d. Always be courteous to all employees and addressed them as Sirs/Ma’ams
e. Mobile phones and other electronic gadgets should be used with discretion
f. If you see fellow CI or student violating guidelines, courteously ask them
to stop or comply
g. Report students violating guidelines to their CI or DH. IRs should be
submitted to the CI supervising them and/or the DH, but not to the faculty
who witnessed the violation
h. Refrain from taking pictures with SNs while on duty especially wearing
uniform; if you cannot help it, make sure it is not posted on social media
i. Gifts from SNs should be graciously declined, but if they insist and you
accepted it, please refrain from posting it in social media as it might elicit
a negative reaction. Just send a TY note or message using the SNs
messenger or GC account.

VIII. EVALUATION OF STUDENTS

a. Make sure students fill-out the worksheets while on duty and manually
comment on the forms as this will form part of their portfolio
b. For 3-days duty, the SN should have 3 manually filled-out worksheets (if
1patient/day); more worksheets if they handled more than 1 patient/day
c. Implement the student/area requirements as it is, please DO NOT make
your own guidelines
d. Immediately check and grade requirements per rotation so as not to
overlap with succeeding rotations
e. Feedback and/or one-on-one counselling with SN is highly encouraged
f. Request evaluation forms from the Dean’s of (printed /photocopy are
likewise accepted)
g. Non-submission of required requirement/s on dates specified would mean
a score of zero, but the SN should still submit the requirement (will be
ungraded) otherwise will be marked as INC
h. Inform SN of incurred violation/s and its corresponding sanctions (ex.
Number of days extension or make up duty)
i. CI signs the endorsement form for make-up/extension on or before the end
of rotation
j. CI should do follow-up and make sure SN rendered the make-up/extension
duty. Update DH regarding completion and/or rotation grade

IX. SUBMISSIONS/DEADLINES

a. Submit grades 5-working days after last day of rotation via email
b. submit Bundy/time card (BGH & BeGH) every 1st day of the month
(Scanned copy/clearly taken via mobile phone is accepted and must be
sent via email to DH)
c. Report absences of students at once to DH
d. Submit IR within 24 hours to DH
e. Rotational exam with TOS must be uploaded in the G-class
f. Submit the following files to DH3:
i. Rotation grade (1-page: fit to page printing, short bond paper)
ii. Filled-out evaluation tools, signed/conformed by all students
iii. Filled out competency checklists for OR / DRN duty
iv. Incident report signed by student/s, with notation of action /
decision from CI
v. Learners’ worksheet & Nurses’ Notes (at least 3-copies for 3-days
