AFFIDAVIT OF LOSS

I, DAN ANGELO G. DUCO , of legal age, Filipino, single, with residence at


Sta. Maria, Moncada, Tarlac, after having been sworn to in accordance with law hereby
depose and say that:

1. I am a Licensed Professional Teacher employed at St. Vincent School


Foundation Inc., Tarlac City;

2. As such, I am a holder of a Professional Regulation Commissions (PRC) ID


issued by the Professional Regulations Commission, Manila;

3. Said PRC ID is always kept inside my wallet;

4. On 26 August 2021, I was surprised to find out that my wallet was gone.

5. My wallet, containing my PRC ID, must have fallen on my way to school as


when I reached our institution and look for my wallet, the same could no longer
be found.

6. I took earnest efforts to look for my wallet and even went back to the gas station
where I remember was the last place I took it out and further went to my way to
office where the same must have fallen but to no avail, hence given up as lost;

7. I execute this affidavit to attest to the truthfulness and veracity of the foregoing
facts, and for the purpose of securing another PRC ID, and for whatever purpose
it may serve.
