AFFIDAVIT OF LOSS

I, HILLARY M. ACEDO, of legal age, single, Filipino citizen, and a


resident of 165, St. Jude, Hipodromo Cebu City, Philippines, after having
been duly sworn in accordance with law, hereby depose and state:

1. That I was issued with a PRC Identification Card (PRC ID) by the
Professional Regulation Commission (PRC);

2. That the said PRC ID is placed inside my wallet;

3. That on October 20, 2019, my wallet was snatched by an unknown


perpetrator;

4. That despite diligent search and effort and even with the help of
police authorities, I can no longer recover my wallet, including my
PRC ID and other important documents;

5. That said PRC ID has not been confiscated, cancelled, expired nor
revoked by PRC ID or any government agency;

6. As such, I am executing this Affidavit of Loss to attest to the truth


of the foregoing and to support my application for the issuance of
a new PRC ID.

IN WITNESS WHEREOF, I have hereunto affixed my signature this


21st day of October 2019 at Cebu City, Philippines.
