B. INSPECTION OF MAIN STRUCTURE YES NO COMMENTS
All standards are in vertical position.

All ledgers are using double couplers.

Are all ledgers connected to all standards?

Sleeve couplers are used for joint tube.

All bracing is correctly installed as per safety in designs


standard provided below*
Main transoms connected to standard with double couplers.

Bay length is not exceeding 2.5 meter (min 1.5 meters).

Ledger/diagonal bracings at each end of scaffold structure are


in Zig-Zag fashion and installed to full height of the structure.

Is longitudinal bracing installed at a 45o angle on both faces of


the scaffold?
Are ledgers erected along the length of the scaffold on both
inside and outside standards at matching elevations?

Are standards accurately spaced, erected on suitable bases?

Safety/protection cap fitted at ends of scaffold tube?

Are outriggers or ties provided for additional support if


working height exceeds four times the least base dimension?
C. INSPECTION OF TIE

The ties are strong and rigid.

The gap between the structure (e.g. tank shell) and scaffold is
no greater than 200mm.
Ties are doubled with 12-gauge iron wire or equivalent and are
spaced not more than 4.8M horizontally and 4.2M vertically.
The ties are as close as possible to the top working platform

D. INSPECTION OF THE WORKING PLATFORM YES NO COMMENTS

The working platform is fully boarded and close-Boarded.

Each board is adequately supported and tied with rope to


minimize gaps between boards.
Are the overhang of the planks between 150mm and 450mm?

Is toe board height minimum of 102 mm (4”)?

Is the vertical gap between the bottom of the toe board and
the surface of the platform, walkway, ramp, deck or floor not
exceeding 6 mm?
Is the horizontal gap between two toe board sections or
between a toe board end and other solid structure, not
exceeding 25 mm?
The platform is level with no lapped, trap or damaged boards.
Intermediate transom must be adequate with Max span not
over 1.5 meter to support the scaffolding boards.(35-38 mm.
Board Thickness)
Top handrails height is 106-110cms, mid rail at 53-55cms.

Toe boards (15.2cms H) are in place at all side of scaffold


structure to prevent materials from falling down.
All load-carrying boards are free of defect such as splitting or
cracking or warping.
Are every board securely lashed to prevent displacement?
Have puncheon erected at the unsupported ends of Toe
Boards?
A screen (18 gauge, 13-mm mesh or equivalent) is installed
between top rail and toe board of platform, where personnel
are required to work or pass under a scaffold platform?
E. INSPECTION OF ACCESS LADDER YES NO COMMENTS
The ladder is on a firm base.

The ladder is of sound construction and completely


undamaged.
They are correctly secured with the proper lashing.

Are ladders and its rungs free from splits, cracks, distortions
and greases?
Access ladder provided to scaffold working levels.

Is the access ladder securely fixed to the scaffold?

Are at least three rungs of ladder above the access platform?

Danger tape in place on ladders that are not in service

Intermediate rest platform is provided when the climbing level


is greater than 9 meters (30 feet)
Are ladders extended a minimum of 305 mm (12”) above the
top rail for handhold during mounting and dismounting?
F. GENERAL REQUIREMENT YES NO COMMENTS
Are staging pieces such as poles, ledgers, bearers, ribbons and
braces are 2 inches (51 mm) outer ø tube steel?
Staging pieces extends minimum 13 mm through couplings to
prevent crimping of ends.
Are Scaffolds and their components capable of supporting four
times the maximum intended load without failure?
Is escape route provided and is kept clear all times?

Are all standards, legs and transoms and securely and rigidly
braced to prevent movement?
Is the scaffold is protected from hot exhaust surface, welding,
burning, riveting, or open flame work?

Are employees prohibited from working on scaffolds during


storms or high winds?

Are employees prohibited from using scaffolds covered with


condensate or oil?
Are wastes, loose materials, and debris removed from
scaffold?
Leaning and over reaching prohibited?

Are Scaffold erectors or user following “Drop Object”


checklist?
G. INSPECTION SCAFFTAG YES NO COMMENTS

Is the scaffold being inspected in the past 7 days?

Is the scaffold safe tag correctly signed and date?


Are scaffolding components free of rust, accidental damage,
oil/chemical spill?
Spare scaffolding materials are stored /stacked away from
work area to prevent Slips, Tips & fall.

H. NOTES
Required Dimensions:

* Key Safety in Designs guideline on tubular steel scaffolding:

12.7.5 Diagonal Bracing


1. The full height of all staging shall be braced. Both the front and back of
longitudinal staging faces shall be braced.
2. Diagonal bracing shall be installed at approximately a 45° angle from near
the base of the first post upward to the extreme top of the scaffold, and shall
be repeated at the base of every fifth longitudinal post.
3. Similarly, diagonal bracing shall be installed from the base of the last post
extending back and upward toward the first post. (See Figures 12.17 for
details)

12.7.6 Cross Bracing


Cross bracing shall be installed across the width of the scaffold at least every
third set of posts horizontally and every fourth ribbon vertically. (See Figures
12.17 for details)
