INSTRUCTIONS
This posit training evaluation instrument is intended to measure how satisfactorily you
trainer has done his job during by whole duration of your training. Please give you honest
rating by checking on the corresponding cell of your response. Your answer will be treated
with utmost confidentiality

trainer prepared and facilitated your training. Please give your honest rating by checking on the

conduct of the training, this shall serve as a checklist of what he/she must do.
