I, Juan T. Dela Crus, of legal age, and a resident of #123, 1st Street, San Miguel, Manila,
plaintiff in the above-entitled case, state under oath that:
1. I have read the above complain and that the allegations therein are true and correct
of my own personal knowledge and/or based on authentic records;
2. The pleading is not filed to harass, cause unnecessary delay or needlessly increase
litigation costs;
3. The pleading’s factual allegations have evidentiary support;
4. I have not previously filed any action involving the same issues in any court,
tribunal, or quasi-judicial agency and that, to the best of my knowledge, no such
action is pending in any of them; and
5. If I should later learn about the filing of such action, I shall report it to the court
within five calendar days from notice.
