TRAINING EVALUATION FORM

Dear Trainees:
The following questionnaire is designed to evaluate the effectiveness of the
Supervised Industry Training (SIT) or On the Job Training (OJT) you had
with the Industry Partners of Damortis National High School. Please check (
) the appropriate box corresponding to your rating for each question asked.
The results of this evaluation shall serve as a basis for improving the design
and management of the SIT to maximize the benefits of the said Program.

General Interpretation:

The workshop layout of the training has an average of 5


which was Outstanding as accordance to CBT requirements. The
interpretation for the CBLM sufficiency has determined that it is
very clear and satisfied. The explanation from objectives of every
training session has identified as very good and very satisfactory.

The summary evaluation of the expected activities and


outputs were determined as very good and very satisfactory

Recommendation:
Keep up the good work and enhance more from the executions of
program sequence.
