AFFIDAVIT OF LOSS TITLE

KNOWN ALL MEN BY THESE PRESENTS:

I, CESAR N. JULIAN SR., of legal age and full civil capacity, Filipino,
resident of Laoag City, Ilocos Norte after being sworn in accordance with law hereby
depose and state THAT:

1. I am one of the surviving heirs of CLEMENTE JULIAN, who is


the registered owner of Lot No. 9861 situated in Juan Luna St Bgy.
19 Sta Marcela Laoag City with OCT No. 9938 issued by Registry
of Deeds of Laoag City containing an area of 323 square meters,
more or less;

2. The title was then entrusted to my father, ANTONIO JULIAN as


we are occupying and in possession of the said property since time
immemorial;

3. The title was later on passed on to me by my father for safekeeping;

4. Sometime in 1960’s, our residential house in the subject property


was infested with termites and damaged some of our documents
including the title of OCT No. 9938 which was pulverized by the
termites;

5. I certify and confirm that the parcel of land covered by OCT No.
9938 was never mortgaged, sold, conveyed, disposed of, or
encumbered, to any person or entity. I certify and confirm further
that said document was neither confiscated nor relinquished in favor
of any government agency or authority;

6. I attest to the truth of the foregoing statements to the best of my own


personal knowledge and based on authentic documents or records.

7. I am executing this Affidavit of Loss Title in support to my


application for a new owner’s duplicate copy of Original Certificate
of Title No. 9938 as well as for all lawful intents and purposes it
may serve.
