AFFIDAVIT OF LOSS

I, CHERYL BEJERANO REOLLOS, of legal age, Filipino citizen and a resident of


Zone 1, Tumaga, Zamboanga City, Philippines, after having been duly sworn to in accordance
with law, hereby depose and say, that:

1. I am the registered owner of that certain motor vehicle with Engine No. MB49395
and Chassis No. MHKM1BF2FBK002545 and as such I was in possession of the
original copies of the Certificate of Registration numbered 169289175 duly issued by
the Land Transportation Office, Zamboanga City, Philippines;

2. Said original copy of Certificate of Registration was kept inside my wallet for
safekeeping;

3. However, I recently discover that the above-mentioned original copy of Certificate of


Registration were no longer inside my wallet and was lost;

4. I exerted diligent efforts to locate and to look for the above-mentioned Certificate of
Registration in other places but the same proved futile and to no avail;

5. The above-described motor vehicle was not sold or mortgage to a third party.

6. For all intents and purposes, I am now declaring the above-mentioned Certificate of
Registration lost and beyond recovery,

7. I execute this Affidavit to attest to the truth of the foregoing facts based on my
personal knowledge for whatever legal purposes it may serve.
