I, JOSE RONALDO MARTINEZ, of legal age, Filipino, and with residential address at
30 Alphine Street. Oro Vista Royale Subd., Baranggay Mayamot, Antipolo City, after having
been duly sworn to in accordance with law, do hereby depose and state that:

1. I am the duly licensed driver of a motor vehicle registered under the name of
MAJESTIC SUNRISE REALM CORPORATION, more particularly described
as follows:

a. MV-FILE NO. : 1301-00000099801


b. PLATE NO. : 130107
c. ENGINE NO. : 2KDS380629
d. MAKE : TOYOTA
e. SERIES : HILUX 2.5L E 4X
f. BODY TYPE : PICK-UP
g. YEAR MODEL : 2014

2. I was issued an ORIGINAL OFFICIAL RECEIPT (OR) AND CERTIFICATE


OF REGISTRATION (CR) for the above-mentioned motor vehicle;

3. Sometime on September 2, 2020, I was looking for the aforementioned


ORIGINAL OFFICIAL RECEIPT (OR) AND CERTIFICATE OF
REGISTRATION (CR) inside our house and the same could no longer be found
and has become irretrievably lost, thus, is now beyond recovery;

4. I exerted conscientious search for the said ORIGINAL OFFICIAL RECEIPT


(OR) AND CERTIFICATE OF REGISTRATION (CR) which, however,
proved futile;

5. I am executing this affidavit to attest to the veracity of all the foregoing facts and
for whatever legal intents and purposes it may serve.
