2. Master equipment checklist available and checked per shift.

3. PPM sticker present on all machines

4. Labeling of non-functioning equipment with job order form.

5. The machines are clean and kept on charging.

B. STORE ROOM

1. Stores room kept clean, organized, closed with good lightning.

2. Room temperature checking and updated.

3. Oxygen cylinder stored in appropriate trolley or chained well.

4. Medical supply expiry date checked.

5. List of Updated PAR level and emergency level clear

C. Patient room

1. Updated bed side label

2. Patient on clean gown.

D. PATIENT INTERVIEW

1. Patient aware about her/his assigned nurse

2. Patient received education regarding fall, pressure ulcer and other


related procedures or medications
3. Nurse respond to call bell immediately

E. ENVIRONMENT SAFETY

1. Clean linen room / clean utility organized, clean and locked.

2. List of Updated PAR level and emergency level clear

3. CSSD expiration date.

4. Dirty linen room /soiled linen inside the humper and organized.

5. Dirty utility rooms kept clean, closed.

6. All Nurses on duty with ID and follow the dress code
