GENERAL HANDLING PRECAUTIONS
This device is intended for use only by qualified medical personnel.
Use only Nihon Kohden approved products with this device. Use of non-approved products
or in a non-approved manner may affect the performance specifications of the device. This
includes, but is not limited to, batteries, recording paper, pens, extension cables, electrode
leads, input boxes and AC power.
Please read these precautions thoroughly before attempting to operate the instrument.

1. To safely and effectively use the instrument, its operation must be fully understood.

2. When installing or storing the instrument, take the following precautions:


(1) Avoid moisture or contact with water, dust, extreme atmospheric pressure, excessive humidity and temperatures,
poorly ventilated areas, and saline or sulphuric air.
(2) Place the instrument on an even, level floor. Avoid vibration and mechanical shock, even during transport.
(3) Avoid placing in an area where chemicals are stored or where there is danger of gas leakage.
(4) The power line source to be applied to the instrument must correspond in frequency and voltage to product
specifications, and have sufficient current capacity.
(5) Choose a room where a proper grounding facility is available.

3. Before Operation
(1) Check that the instrument is in perfect operating order.
(2) Check that the instrument is grounded properly.
(3) Check that all cords are connected properly.
(4) Pay extra attention when the instrument is in combination with other instruments to avoid misdiagnosis or other
problems.
(5) All circuitry used for direct patient connection must be doubly checked.
(6) Check that battery level is acceptable and battery condition is good when using battery-operated models.

4. During Operation
(1) Both the instrument and the patient must receive continual, careful attention.
(2) Turn power off or remove electrodes and/or transducers when necessary to assure the patient’s safety.
(3) Avoid direct contact between the instrument housing and the patient.

5. To Shutdown After Use


(1) Turn power off with all controls returned to their original positions.
(2) Remove the cords gently; do not use force to remove them.
(3) Clean the instrument together with all accessories for their next use.

6. The instrument must receive expert, professional attention for maintenance and repairs. When the instrument is
not functioning properly, it should be clearly marked to avoid operation while it is out of order.

7. The instrument must not be altered or modified in any way.

8. Maintenance and Inspection


(1) The instrument and specified parts must undergo regular maintenance inspection at the interval which is specified
after the GENERAL HANDLING PRECAUTIONS section.
(2) If stored for extended periods without being used, make sure prior to operation that the instrument is in perfect
operating condition.

(3) Technical information such as parts list, descriptions, calibration instructions or other information is available for
qualified user technical personnel upon request from your Nihon Kohden representative.

9. When the instrument is used with an electrosurgical instrument, pay careful attention to the application and/or
location of electrodes and/or transducers to avoid possible burn to the patient.

10. When the instrument is used with a defibrillator, make sure that the instrument is protected against defibrillator
discharge. If not, remove patient cables and/or transducers from the instrument to avoid possible damage.

WARRANTY POLICY
Nihon Kohden Corporation (NKC) shall warrant its products against all defects in materials and workmanship for one year
from the date of delivery. However, consumable materials such as recording paper, ink, stylus and battery are excluded
from the warranty.

NKC or its authorized agents will repair or replace any products which prove to be defective during the warranty period,
provided these products are used as prescribed by the operating instructions given in the operator’s and service manuals.

No other party is authorized to make any warranty or assume liability for NKC’s products. NKC will not recognize any
other warranty, either implied or in writing. In addition, service, technical modification or any other product change
performed by someone other than NKC or its authorized agents without prior consent of NKC may be cause for voiding
this warranty.

Defective products or parts must be returned to NKC or its authorized agents, along with an explanation of the failure.
Shipping costs must be pre-paid.

This warranty does not apply to products that have been modified, disassembled, reinstalled or repaired without Nihon
Kohden approval or which have been subjected to neglect or accident, damage due to accident, fire, lightning, vandalism,
water or other casualty, improper installation or application, or on which the original identification marks have been
removed.

In the USA and Canada other warranty policies may apply.

United States law restricts this product to sale by or on the order of a physician.

EMC RELATED CAUTION
This equipment and/or system complies with IEC 60601-1-2 International Standard for electromagnetic
compatibility for medical electrical equipment and/or system. However, an electromagnetic environment that
exceeds the limits or levels stipulated in IEC 60601-1-2, can cause harmful interference to the equipment
and/or system or cause the equipment and/or system to fail to perform its intended function or degrade
its intended performance. Therefore, during the operation of the equipment and/or system, if there is any
undesired deviation from its intended operational performance, you must avoid, identify and resolve the
adverse electromagnetic effect before continuing to use the equipment and/or system.

The following describes some common interference sources and remedial actions:

1. Strong electromagnetic interference from a nearby emitter source such as an authorized radio station
or cellular phone:
Install the equipment and/or system at another location. Keep the emitter source such as cellular phone
away from the equipment and/or system, or turn off the cellular phone.

2. Radio-frequency interference from other equipment through the AC power supply of the equipment
and/or system:
Identify the cause of this interference and if possible remove this interference source. If this is not
possible, use a different power supply.

3. Effect of direct or indirect electrostatic discharge:


Make sure all users and patients in contact with the equipment and/or system are free from direct or
indirect electrostatic energy before using it. A humid room can help lessen this problem.

4. Electromagnetic interference with any radio wave receiver such as radio or television:
If the equipment and/or system interferes with any radio wave receiver, locate the equipment and/or
system as far as possible from the radio wave receiver.

5. Interference of lightning:
When lightning occurs near the location where the equipment and/or system is installed, it may induce
an excessive voltage in the equipment and/or system. In such a case, disconnect the AC power cord
from the equipment and/or system and operate the equipment and/or system by battery power, or use
an uninterruptible power supply.

6. Use with other equipment:


When the equipment and/or system is adjacent to or stacked with other equipment, the equipment
and/or system may affect the other equipment. Before use, check that the equipment and/or system
operates normally with the other equipment.

7. Use of unspecified accessory, transducer and/or cable:


When an unspecified accessory, transducer and/or cable is connected to this equipment and/or system,
it may cause increased electromagnetic emission or decreased electromagnetic immunity. The specified
configuration of this equipment and/or system complies with the electromagnetic requirements with the
specified configuration. Only use this equipment and/or system with the specified configuration.

8. Use of unspecified configuration:


When the equipment and/or system is used with the unspecified system configuration different than
the configuration of EMC testing, it may cause increased electromagnetic emission or decreased
electromagnetic immunity. Only use this equipment and/or system with the specified configuration.

9. Measurement with excessive sensitivity:


The equipment and/or system is designed to measure bioelectrical signals with a specified sensitivity. If
the equipment and/or system is used with excessive sensitivity, artifact may appear by electromagnetic
interference and this may cause mis-diagnosis. When unexpected artifact appears, inspect the
surrounding electromagnetic conditions and remove this artifact source.

10. Use with radiation therapy equipment:


When the equipment and/or system is used in a radiotherapy room, it may cause failure or malfunction
due to electromagnetic radiation or corpuscular radiation. When you bring the equipment and/or system
into a radiotherapy room, constantly observe the operation. Prepare countermeasures in case of failure
or malfunction.

If the above suggested remedial actions do not solve the problem, consult your Nihon Kohden
representative for additional suggestions.

For EMC compliance, refer to “Specifications - Electromagnetic Compatibility” in the General section.

The CE mark is a protected conformity mark of the European Community. Products with the CE mark comply
with the requirements of the Medical Device Directive 93/42/EEC.

The CE mark is only applied to MEB-9400K.

MEB-9400A complies with International Standard IEC 60601-1-2: 2001 and Amendment 1: 2004.
MEB-9400K complies with International Standard IEC 60601-1-2: 2007.

Introduction

*1 Essential performance in EMC The Neuropack MEB-9400A EMG/EP Measuring System measures and averages
standard EMG, electric/auditory/visual evoked potential and nerve conduction.*1

*2 Essential performance of the EMG/ The Neuropack MEB-9400K EMG/EP measuring system displays information
EP measuring system from continuous or intermittent somatosensory, auditory and visual evoked
potential measurement, EMG and EEG measurement, and limb temperature to
support neuromuscular and brain function testing*2 in the examination room,
OR, ICU and other areas.

This service manual provides useful information to qualified service personnel


to understand, troubleshoot, service, maintain and repair the MEB-9400A/K,
MEB-9402C or MEB-9404C EMG/EP Measuring System (referred to as
“system” or “MEB-9400 system” in this service manual).

All replaceable parts or units of this system are clearly listed with exploded
illustrations to help you locate the parts quickly.

The maintenance section in this service manual only describes the maintenance
that should be performed by qualified service personnel. The maintenance

Service Policy, Service Parts and Patient Safety Checks

Service Policy
Our technical service policy for this system is to replace the faulty unit, board
or part or damaged mechanical part with a new one. Do not perform electrical
device or component level repair of the multilayer board or unit. We do not
support component level repair outside the factory for the following reasons.
• Most of the boards are multilayer boards with surface mount electrical devices,
so the mounting density of the board is too high.
• A special tool or high degree of repair skill is required to repair the multilayer
boards with surface mount electrical devices.

Only disassemble the system or replace a board or unit in an environment where


the system is protected against static electricity.

As background knowledge for repair, take the following into account.


• You can reduce the repair time by considering the problem before starting
repair.
• You can clarify the source of most of the troubles using the information from
the error massage and troubleshooting. Refer to the “Troubleshooting and
Error Messages” section in this manual.

Service Parts
Refer to the “Replaceable Parts List” section in this manual for the service parts
for technical service that we provide.

NOTE
When ordering parts or accessories from your Nihon Kohden
representative, please quote the code number and part name which is
listed in this service manual, and the name or model of the unit in which
the required part is located. This will help us to promptly attend to your
needs. Always use parts and accessories recommended or supplied by
Nihon Kohden Corporation to assure maximum performance from your
system.

Nihon Kohden Corporation (NKC) shall stock repair parts (parts necessary to
maintain the performance of the instrument) for a period of 8 years (PC unit: 4
years) from the date of delivery. In that period, NKC or its authorized agents will
repair the instrument. This period may be shorter than 8 years (PC unit: 4 years)
if the board or part necessary for the faulty section is not available. After 4 years,
if the PC unit becomes faulty, replace it with a specified one.

Patient Safety Checks 1


Periodic maintenance procedures and diagnostic check procedures are provided
in this manual to ensure that the system is operating in accordance with its design
and production specifications. To verify that the system is working in a safe
manner with regard to patient safety, patient safety checks should be performed
on the system before it is first installed, periodically after installation, and after
any repair is made on the system.

For patient safety checks, perform the following checks as described in the
IEC 60601-1 “Medical electrical equipment - Part 1: General requirements for
safety”.
• Earth leakage current check: 0.5 mA or less
• Enclosure leakage current check: 0.1 mA or less
• Patient leakage current check: 0.1 mA or less

Maintenance Equipment/Tools
Test equipment
When repairing or calibrating the system, the following test equipment is
required.
• Oscilloscope
2 channels or more for input signal, 50 mV to 5 V input range, 1/10 attenuating
probe and 100 MHz or more frequency response characteristic must be provided.
• Power supply

General Information on Servicing

Note the following information when servicing the system.

CAUTION
Safety
• There is the possibility that the outside surface of the system, such
as the operation keys, could be contaminated by contagious germs,
so disinfect and clean the system before servicing it. When servicing
the system, wear rubber gloves to protect yourself from infection.
• There is the possibility that when the lithium battery is broken, a
solvent inside the lithium battery could flow out or a toxic substance
inside it could come out. If the solvent or toxic substance touches
your skin or gets into your eyes or mouth, immediately wash it with a
lot of water and see a physician.

Liquid ingress
The system is not waterproof, so do not install the system where water
or liquid can get into or fall on the system. If liquid accidentally gets into
the system or the system accidentally drops into liquid, disassemble
the system, clean it with clean water and dry it completely. After
reassembling, verify that there is nothing wrong with the patient safety
checks and function/performance checks. If there is something wrong
with the system, contact your Nihon Kohden representative for repair.

Environmental safeguards
Depending on the local laws in your community, it may be illegal to
dispose of the lithium battery in the regular waste collection. Check
with your local officials for proper disposal procedures.

Disinfection and cleaning


To disinfect the outside surface of the system components, wipe
them with a nonabrasive cloth moistened with any of the
disinfectants listed below. Do not use any other disinfectants or
ultraviolet rays to disinfect the system components.

• Use the specified shipment container and packing material to
transport the system. If necessary, double pack the system. Also,
put the system into the shipment container after packing so that
the buffer material does not get inside the system.
• When transporting a board or unit of the system, be sure to use
a conductive bag on. Never use an aluminum bag to transport a
board or unit which a lithium battery is mounted. Also, never use
a styrene foam or plastic bag which generates static electricity to
wrap the board or unit of the system.

Handling the system


• Because the outside surface of the system is made of resin,
the outside surface of the system is easily damaged. So when
handling the system, remove clutter from around the system and
be careful to not damage the system or get it dirty.
• Because most of the boards in the system are multilayer boards
with surface mount electrical devices (SMD), when removing and
soldering the electrical devices, a special tool is required. To avoid
damaging other electrical components, do not remove and solder
SMD components yourself.

Measuring and test equipment


Maintain the accuracy of the measuring and test equipment by checking
and calibrating it according to the check and calibration procedures.

Communications Equipment and the MEB-9400A/K
The MEB-9400A/K is intended for use in an electromagnetic environment in which radiated RF disturbances are
controlled. The customer or the user of the MEB-9400A/K can help prevent electromagnetic interference by maintaining a
minimum distance between portable and mobile RF communications equipment (transmitters) and the MEB-9400A/K as
recommended below, according to the maximum output power of the communications.

Overview
Error messages are displayed when an error occurs in the MEB-9400 system
during operation or in the power on self check for the main unit. Some of the
display error messages have a “Command code” and “PC-ACQ API error
code” which will be the key to solve the problem. When an error is detected in
the power on self check for the main unit, the PC-ACQ API error code has an
“additional information” code.

Command code: Command code communicated between the PC unit and


main unit when an error occurred.

PC-MPU error code: Error code which indicates the cause of the error
message.

Additional information This information consists of 32 bits (hex) and indicates


code: which power on self check result caused the error
message to be displayed.

Additional Information List


Following is an explanation of the meaning of the additional information code.

The “additional information” is a 32 bit hex code which is displayed with the
Command code and PC-ACQ API error code when the initialization program
in the flash memory detects an error during the power on self check of the main
unit. The bit combination indicates the problem.

The leftmost bit of the additional information is bit 31; the rightmost bit is bit 0.
In the following example, bit 31 is 9 and; bit 0 is 1.

• When bits 0 to 13 are 0, no errors are present and bits 14 to 31 are also 0.

This item checks the ROM by comparing the lowest single byte of the sum value
between all data with the exception of the last address (0x1fff) value and 0xff
value with the 0x1ffff value. If the values are not the same, an error message
with the additional information (bits 31 to 28: 9001) is displayed.

If this error message appears, the ROM in the main unit is faulty.

RAM Error
This item checks the RAMs by comparing the test patterns that are written to the
RAM with the test patterns that are read from it. If there is a difference, an error
message with the additional information (bits 31 to 28: 9002) is displayed. The
checked RAMs are the main RAM for MPU and share memory for MPU and PC
unit.

If this error message appears, the RAMs in the main unit are faulty.

Interval Timer Error


This item checks if an interrupt request signal for internal timer correctly
generates or not. If the interrupt request signal does not generate correctly, an
error message with the additional information (bits 31 to 28: 9004) is displayed.

If this error message appears, the MPU in the main unit is faulty.

DSP Error
This item checks if the communication between the DSP and MPU in the main
unit works correctly or not. If the communication does not work correctly, an
error message with the additional information (bits 31 to 28: 9005) is displayed.

Startup Sequence

The PC unit controls the entire MEB-9400 system by communicating with the 3
main unit. When the power is turned on, the PC unit downloads the main unit
operation programs to the main unit.

Main Unit

The main unit consists of the main board, panel board, temp board* and jack
board*. The main board acquires the waveform from an electrode junction box,
processes the waveform with the digital signal processor, and controls the trigger,
auditory visual stimulation blocks and temperature measurement block*. The
main unit communicates with the PC unit by USB communication.

Main Board
The main board consists of the following blocks.
• MPU block
Controls the main unit, electrode junction box.

• DSP block
Processes the waveform data from the electrode junction box.

• USB block
Communication between the MPU and PC unit is performed via the USB
controller.

• Parallel to serial converter


Converts the parallel data to serial data and sends it to the electrode junction
box, electrical stimulation block and auditory stimulation block.

• Trigger block
The trigger block consists of the trigger control block and stimulation interface
block. The trigger control block generates trigger signals for the electrical,

Power Unit
The power required for the MEB-9400 system is supplied from the power unit.
The power unit generates +15 V DC power with the switching regulator and
supplies it to the main board. The power block on the main unit generates the
following DC voltages from the +15 V DC power.

Temp Board (MEB-9400A only)


The temperature measurement voltage divided by the thermistor resistor passes
through the 1.6 Hz high-cut filter. The temperature measurement, calibration and
reference voltages are sent to the multiplexer. Voltage is converted to a resistance
value with the bridge circuit whose reference voltage is 1.0 V.

The temperature measurement control signal from the main board is latched in
the non-isolated circuit and sent to the multiplexer through a photocoupler. This
signal changes the multiplexer switch every second to select the voltage from the
temperature measurement, calibration and reference voltages.

Jack Board (MEB-9400A only)


The jack board has a connector to connect the thermistor probe and sends signals
from the thermistor probe to the temp board.

Function Outline
• Signal input circuit
Signals are input from the eight electrode jacks (JB-944BK, four electrode jacks:
JB-942BK) for disk electrodes and four electrode lead jacks (JB-944BK, two
electrode lead jacks: JB-942BK). Each input circuit provides 200 MΩ resistor 3
as input impedance. Zener diodes and arrestors are provided for protecting
the input circuit from excessive voltage caused by static energy or electrical
surgery unit. A 16 Hz, 8 Vp-p skin-electrode contact impedance measurement
signal is applied to the patient.

• Skin-electrode contact impedance measurement signal distributor


Distributes 16 Hz, 8 Vp-p sine-wave for the skin-electrode impedance
measurement which is generated in the skin-electrode contact impedance
signal generator.

• Differential amplifier
Acquired waveform is amplified about 32 times. The buffer amplifier for
common mode component feedback is connected to pin 11 (COM) of each
differential amplifier. The buffer amplifier for shield feedback is connected to
pin 10 and pin 12 (–FB, +FB) of each differential amplifier.

• Low cut filter


0.01, 0.5, 1 and 10 Hz (–6 dB/oct) low cut analog filter consists of a capacitor
and resistors. The other low cut filters are realized by digital processing with
the DSP on the main board.

• Fast Recovery circuit


When the FRCVf signal for the fast recovery is sent from the MPU, the time
constant of the low cut filter is longer.

• Reset circuit
When the RESET button on the Tool bar on an examination window is clicked,
the input of the second stage amplifier is connected to the floating ground.

• Second stage amplifier


The acquired waveform goes through a 48 kHz high-cut filter and is amplified
×4 or ×8 by the non-inverting amplifier. When “Sens” of the Amplifier page of
the Set Conditions window is set to 5 mV or 10 mV/div, the amplitude is set to
×4.

• Attenuator
The attenuation ratio (×1, ×1/5, ×1/50, ×1/500, ×1/1250) is set according to
“Sens” setting of the Amplifier page of the Set Conditions window.

• Third stage amplifier

• Fourth stage amplifier


About ×16 non-inverting amplifier. The total gain is adjusted by VR041, 042,
051 and 052 (JB-944BK) by VR041 and 042 (JB-942BK). Refer to “Signal
Input Circuit Check/Adjustment” in Section 4.

• Anti-aliasing filter
High cut frequency is 5, 10 or 30 kHz (–18 dB/oct). The high cut frequency
automatically changes according to the number of channels used.

• A/D converter
Converts the multiplexed four channel analog signal to digital data. The A/D
converter is 16-bit, 10 μs sampling, serial type A/D converter. The multiplexer
and A/D converter are controlled by the signals from the main unit. The
converted digital data is sent to the DSP on the main unit at the 6.6 MHz
transfer rate.

• Serial to parallel data converter


Converts the serial data from the main unit to control the status of the signal
processing circuits and skin-electrode contact impedance measurement signal
generator.

• Skin-electrode impedance measurement signal generator


Generates the 16 Hz, 8 Vp-p sine wave for measuring the skin-electrode
impedance.

• Impedance check indicator


The impedance check LED shows the result or the skin-electrode contact
impedance check. A green lit LED indicates an electrode impedance is equal to
or less than the preset value. The impedance threshold LED shows the selected
impedance threshold.

• Optical isolator
Four high-speed type photocouplers are used for transferring the digital
data from the floating circuit to the non-floating circuit. The other three
photocouplers are used for the serial communication between the signal input
block and receiving the status signal from the main unit.

• Floating power block


Receives the 250 kHz clock from the main unit with the flip-flop circuit then
drives the pulse transformer with the two FETs to generate floating power. The
±17.4 V and ±5 V are generated by the three terminal regulators on the floating
circuit. If either of plus or minus regulator is damaged, the power supply for
the floating circuit is automatically stopped.

If the periodic inspection is not performed, degradation or loss of function may


go unnoticed and lead to misdiagnosis.

Service personnel should perform the periodic inspection at least once every
year. Make sure that the EMG/EP measuring system operates properly and
replace the consumables.

If you found abnormalities as a result of inspection and the EMG/EP measuring


system is suspected to be faulty, attach an “Unusable” or “Repair request”
label to the EMG/EP measuring system and contact your Nihon Kohden
representative.

For periodic inspection, use the Maintenance Check Sheet in this section. This
check sheet is useful to check the operation of the system after check/adjustment
or board/unit replacement.

Power Unit Check

Measure the output voltage from the 3-prong AC outlet on the power unit after
power on and make sure that the measured voltage is within ±10% of the line
voltage.

If the measured voltage is out of range, troubleshoot by following “Problems

The procedures in this section explain how to remove, replace and install major
components in the system.
• To remove the components from the KD-026A Cart or KD-019A Cart, refer to
the MEB-9400 operator’s manual.
• To remove, replace and install the components in the PC unit, refer to the
personal computer documentation.
• To remove the connection cables, refer to “Cable Connection” in Section 2 of
the MEB-9400 operator’s manual.

1. Turn the main unit upside down.

2. Remove the seven M3 × 8 binding head screws with toothlock washers from
the bottom cover and remove the bottom cover. The main board and power
unit are attached to the bottom cover.
