MEMORANDUM OF AGREEMENT 

This agreement entered into and executed this ________ day of __________________ 2022
at Bacolod City, Negros Occidental, and Philippines by and between. 
The PROVINCE OF NEGROS OCCIDENTAL with principal office at the Old Capitol 
Building Road, Bacolod City represented hereto by Governor EUGENIO JOSE V. LACSON 
hereinafter to as the GRANTOR; 
__________________________ Filipino and a resident of ____________________, NEGROS OCCIDENTAL
together with ______________________ his/ her parent/ guardian likewise Filipino and a
resident of _________________, NEGROS OCCIDENTAL herein after referred to as the
GRANTEE. 
WITNESSETH 

WHEREAS, the Provincial Government of Negros Occidental recognizes the 


contribution of the poor and deserving youth in the growing change and development of
the  rural areas in the province of Negros Occidental; 
WHEREAS, these rural youth, the poor and the deserving college students
presently enrolled, wish to pursue their studies; 
WHEREAS, the Governor EUGENIO JOSE V. LACSON EDUCATIONAL
ASSISTANCE PROGRAM, is one opportunity for the rural youth to become productive
members of the community whose skills and potential, when developed and harnessed will
make a difference in improving the quality of life in their respective families and the
community as a whole. 
WHEREAS, in order to operationalize this program, the screening committee has
set guidelines for the program; 
WHEREAS, said guidelines cover the qualifications rights and privileges, benefits
and obligations of the parties. 
NOW, THEREFORE, and in consideration of the foregoing premises, the parties
hereto agree as follows; 
SECTION 1: OBLIGATION OF THE PARTIES 

1. The Grantor shall: 


a. Ensure the availability of the funds of the program. 
2. The Grantee shall:
a. Maintain a general average of 2.5/ 80% or its equivalent with no failing grades
or dropped subjects. In case of failure to comply these requirements, grantee shall
be dropped from the lists of scholars for the succeeding semester; 
b. Complete all requirements of the study within the period specified.
c. Follow the rules and regulations of the school where he/ she is
enrolled. 
d. Serve the community where he/ she came from and must initiate any 
entrepreneurial activities. 
e. Comply all obligations stated herein. If he/ she fail to do so the
Provincial Government will impose appropriate sanctions. 
SECTION 2: EFFECTIVITY 

This agreement shall be binding upon the parties and will take effect
on _________________________________. 
SECTION 3: AMENDMENT 

This agreement may be amended and terminated subject to the guidelines set forth
by the screening committee or upon the agreement of both parties. 
IN WITNESS THEREOF, the parties have hereto affixed signature on
