PARTS OF EXTERNAL FEMALE GENITALIA

DEFINITION
➢The aseptic cleansing of the vulva and
perineum.
ASSESSMENT
1. Review general assessment data about patient.
2. Observe for signs of perineal itching, burning on
urination, or skin irritation. Ask patient if he or she
experiences any of these problems.
ASSESSMENT
4. Assess patient's ability to bathe herself and to perform
perineal care.

5. Reassess patient's learning needs related to perineal care,


while providing privacy, assess the perineal/genital area for
abnormalities.
OBJECTIVES
1. To keep the vulva and perineum clean.
2. To promote healing of perineal wound.
3. To prevent infection.
4. To prevent disagreeable odor.
5. To remove secretions.
6. To provide comfort.
GENERAL CONSIDERATIONS
1. Flushing water should be of the right temperature to
avoid burning the patient.
2. Observe special care when a patient has a
perineal wound or stitches to avoid much discomfort.
3. If the patient has defecated or voided, empty
the bedpan first before giving perineal.
4. Discard used perineal balls into the waste
receptacle, not into the bedpan.
EQUIPMENT

PROCEDURE
STEPS
1. Prepare and gather equipment.
2. Explain the procedure and its purpose.
3. Screen the patient.
4. Remove perineal pad, if any. Wrap it with paper and discard into
waste can.
5. Wash hands and wear gloves.
6. Loosen the foot part of the top sheet and place patient in dorsal
recumbent position.
7. Place rubber protector under patient's buttocks.
8. Place the patient on bedpan. Fan fold the top sheet
towards the knees, to expose vulva and perineum. Put
a bath towel over the abdomen.
9. Place lined kidney basin outside the tray and position it
conveniently for use.
10. Pour sterile/tap water over the vulva.
11. Clean the vulva and perineum using perineal balls
soaked in soap sud solution. Use a downward stroke in
the following sequence:
a.Center of vulva
b.Farther labia minora
c.Nearer labia minora
d.Farther labia majora
e.Nearer labia majora
f. Center of vulva down to the
anus
g. Mons veneris/mons pubis (zigzag motion)
h. Farther thigh (sunshine motion)
i. Nearer thigh (sunshine motion). Use a new or
clean perineal ball for each stroke. Repeat
sequence of cleansing as necessary.
12. Discard used perineal balls into the waste receptacle.

13. Rinse area with warm water. See to it that the soap
solution is completely rinsed off.
14. Dry vulva and perineum with dry perineal balls following
the same sequence in cleansing.
15. Remove bedpan and turn patient to side near you or
health care provider.
16. Dry buttocks and anus using tissue paper.
18. Assist patient to assume a dorsal recumbent
position. Apply medication as ordered. And perineal
pad, if necessary.
19. Remove the bed protector (optional).
20. Arrange the beddings and make patient comfortable.

21. Do after care. Remove gloves and wash hands.


EVALUATION
1. Perineal care has been comfortably and
effectively provided.

2. Perineal area is clean, odor-free.
