1. GENERAL

This procedure covers safe work practices and steps to safeguard employees and equipment in
a place of work from the hazards of energy sources during offshore campaign, operational and
maintenance activities.
It requires, in part, turns off and disconnects the machinery or equipment from its energy
source(s) before performing testing, commissioning, service or maintenance by lock and tag the
energy-isolating device(s) to prevent the release of hazardous energy and take steps to verify
that the energy has been isolated effectively.
This document will be supported by JSC Health, Safety and Environment documents which
where applicable.

2. SCOPE

This procedure are applicable to any work situation where energy sources may pose a hazard to
workers, equipment and or the environment. Energy sources may include but are not limited to
the following: electrical, mechanical, hydraulic, thermal, chemical, pneumatic, ionizing radiation
etc.

4. DEFINITION AND ABBREVIATIONS

TERM DEFINITION
Approving Authority (AA) : The person authorised under PTW system to approve
and issue PTW in the area concerned. Work involving
increased or unusual hazards such as work involving
open flame, hot tapping or entry into a confined space,
shall require review and authorization at a higher level. As
such the AA shall ensure that any equipment being
worked on shall be isolated and made safe of any energy
hazard.
Approving Authority : The person authorised by the Approving Authority (AA) to
Representative (AAR) carry out safety precautions prescribed in the
permits/certificates prior to commencement of work. This
person must also be qualified and whose job requires
him/her to operate or use a machine or equipment on
which servicing or maintenance is being performed under
lockout or tag out, or whose job requires him/her to work

in an area in which such servicing or maintenance is


being performed.
Authorised Competent : A person also referred to as the Chargeman, who is
Electrical Person (ACP) registered with Energy Commission and authorised by
Management. He has sufficient technical knowledge and
experience to enable him to avoid danger and who can
have authority delegated to him, according to his
knowledge and experience to carry out operations in
power systems and work on electrical equipment. He has
the authority to issue and cancel Electrical Isolation
Certificate and Limit of Electrical Access Certificate.
Appointed Person (AP) : A person, especially trained for certain duties (e.g.
isolation/de-isolation of electrical equipment,
synchronizing etc.) and has sufficient technical
knowledge and experience to enable him to avoid danger.
For him to execute the works he MUST be under written
Instructions and Appointment from Resident Electrical
Engineer as provision in Electricity Regulations 1994. For
the Appointment to take effect, it must be endorsed by
Management.
Blank Flanges/Capping : A plate or cap securely attached to the flange point at the
end of a pipe. The cap or blank shall be made of material
that is compatible with the liquid, gas or vapor with which
they are in contact. The material for the cap or blank shall
have sufficient strength to withstand the maximum
operating pressure, which can build up in the piping. This
term is also referred to as end blind.
Blinding / Spadding : Blank plates inserted in a flanged joint between the
equipment and the rest of the plant or the source of
hazardous energy. Blinds / Spades or plates must be
capable of withstanding the maximum design pressure,
which may build up in the pipe. These are known as “on spec
blinds” or “spec blinds”. Blinds made of thin plate, not able to
withstand the design process pressure are known as “off
spec blinds”.
Double Block and Bleed : Double block and bleed is a method that is used to isolate
energy away from intended work. This is achieved by
having two independent closed valves separated by
some distance with a bleed valve between them in the
open position. The function of the bleed may also be
achieved by the use of a valve with a functioning body
bleed in the open position. Refer to Appendix 6.
Energized : Connected to an energy source or containing residual or
stored energy.
Energy-isolating Device : A mechanical device that physically prevents the
transmission or release of energy, including but not
limited to the following: a manually operated electrical
circuit breaker; a disconnect switch; a manually operated
switch by which the conductors of a circuit can be
disconnected from all ungrounded supply conductors,

and in addition, no pole can be operated independently; a


line valve; a block valve; and any similar device used to
block or isolate energy. Push buttons, selector switches
and other control circuit-type devices are not
energy-isolating devices.
Energy Source : Any source of electrical, mechanical, hydraulic,
pneumatic, chemical, thermal, ionizing radiation or any
other form of energy.
Isolation/Lockout Device List : A controlled document approved by the department
management, which indicates all isolation and lockout
device locations required to safeguard against energy
hazards. Refer to Appendix 5, 6 & 7.
Lock Tag Clear & Try : After all switches, valves, and devices are locked out and
tagged, and the equipment is cleared of personnel, the
supervisor ensures the effectiveness of the lockout by
witnessing the operation of the push button, actuators,
etc., to confirm that the equipment will not start. The
equipment has now been locked out.
Lockout : The placement of a lockout device on an energy isolating
device, in accordance with an established procedure,
ensuring that the energy isolating device and the
equipment being controlled, cannot be operated until the
lockout device is removed.
Lockout Box : This is a secure box capable of holding the keys from
lockout devices used to render isolation points in a
system inoperable. The box must be equipped with a
visible means of observing the keys inside and has
provision to be secured by additional locks. The Lockout
Boxes shall be located in the Permit to Work issuing area.
Refer to Appendix 7.
Lockout Device : A device such as a clamp and lock that uses a positive
means to hold an energy isolating device in the safe
position and prevent the energizing of a machine or
equipment. Lockout devices which are used for electrical
isolation of the MCC shall have individual numbers that
will be used for recording on the PIC and EIC.
Physical Isolation Certificate : The PIC is a certificate related to the Permit to Work
(PIC) system that is used to record the condition of equipment
requiring energy isolation. It is classed as a Special
Certificate and may stand without a Permit to Work for
purposes of long term isolation. PIC is shown in Appendix
1.
Isolation of more than 6 months shall be carried out via
Management of Change (MOC).
Positive Isolation : Positive isolation means the equipment or system being
isolated is secured from the rest of the process or system
using certain isolation devices. Isolation by these devices
may be accomplished by any or all of the following:

• Disconnection of piping
• Removed valves
• Removed spool piece
• Removed spacer or expansion joint in the piping leading to
the equipment or system to be worked on
• Installation of a blank flange or cap on one or both of the
open ends.
• Insertion of spades or plates capable of withstanding the
maximum design pressure or swinging closed a full pressure
spectacle blind in the piping between the equipment and the
rest of the plant or the source of hazardous material. In
certain cases where the on spec blind/spades cannot be
inserted, off spec blinds/spades may be used for positive
isolation. A risk assessment must be performed before off
spec blinds/spades are used.
Isolation of Ionizing : Ionizing radiation sources such as nucleonic level
Radiation detectors may exist in some PETRONAS facilities. These
radioactive sources must be isolated, shuttered or
removed as part of preparation for confined space work or
work that may expose workers to ionizing radiation.
Radiation Protection : An individual duly approved by the Atomic Energy
Officer/Supervisor Licensing Board and authorized in writing by the
(RPO/RPS) contractor or OPU management. He shall also be the
competent person of the contractor or OPU.
Receiving Authority (RA) : A person authorized by PFLNG2 to receive approved
permits and shall be responsible for supervising the
execution of the work by Work Leaders as per the
precautions in the permit. The Receiving Authority (RA)
can also be the Work Leader (WL). The RA shall specify
controls for safe work and requirements of other
certificates. The RA shall also be responsible to ensure
that the equipment being worked on is correctly isolated,
and the lock out and tag out has been completed before
the commencement of servicing or maintenance on the
equipment.
Receiving Authority : The person authorised by the Receiving Authority (RA) to
Representative (RAR) ensure all the safety precautions prescribed in the
permits/certificates are carried out prior to
commencement of work.
Construction and/or : Workplace activities such as constructing, installing, setting
maintenance up, adjusting, inspecting, modifying, and maintaining and/or
servicing machines or equipment. These activities include
lubricating, cleaning or unjamming machines or equipment
and making adjustments or tool changes where the
employee may be exposed to the unexpected energization
or startup of the equipment or release of hazardous energy.
Setting up : Any work performed to prepare a machine or equipment
to perform its normal production operation.
Spectacle Blind / Spade : Two symmetrically arranged spectacle frames one of

which is blank plate and the other as an open frame with a


hole the dimension of which is usually sized to the full
bore of the piping in which it is installed. The spectacle is
attached to the pipe or flange and hinged in such a way to
allow the spectacle to be swung to the blank or the open
position when the flange is loosened.
Tag out : The placement of a tag on an energy isolating or lock out
device, in accordance with an established procedure, to
indicate that the energy-isolating / lock out device and the
equipment being controlled by it may not be operated until
the tag is removed.
Tag : This is a prominent warning device that an authorised
employee fastens to energy-isolating devices to warn
other employees that the energy-isolating device and the
equipment being controlled may not be operated until the
tag is removed. The tag must have a means of
attachment, which can be securely fastened to an
energy-isolating device in accordance with an established
procedure.

5. RESPONSIBILITY

5.1 APPROVING AUTHORITY (AA)


The AA is also responsible for approving the PTW for commissioning, construction and/or
maintenance work on equipment or systems where energy isolation is required. This includes
assessing the nature of work and its associated hazards. He shall ensure that the work has a
thorough Job Method Statement (JMS) and/or JSA resulting in a safe system of work. He is also
responsible to ensure that the scope of work is correctly defined in the Permit to Work and
control measures for identified hazards are in place. He has to ensure only trained and
competent personnel conduct the preparation and isolation activities.

5.2 APPROVING AUTHORITY REPRESENTATIVE (AAR)


The area operator/technician appointed by the Approving Authority to prepare and isolate the
equipment. He / she may also be instructed to install any lockout devices and tags under the

supervision of the Approving Authority. The AAR shall be competent to operate the equipment
and understand the process system being isolated.

5.3 RECEIVING AUTHORITY (RA)


The person responsible for supervising the maintenance activities who must ensure that the
system or equipment being worked on has been safely isolated and disabled with proper lockout
devices and tags. The RA will receive the PTW issued for work to be carried out under his
supervision.

5.4 RECEIVING AUTHORITY REPRESENTATIVE (RAR)


The person usually responsible for conducting the maintenance activities who must also
confirms that the system or equipment being worked on has been safely isolated and disabled
with proper lockout and tags.

5.5 APPOINTED PERSON (AP)


The AP is responsible to complete the physical isolation or electrical isolation as requested by
the AA. He shall also complete the EIC and PIC recording the tag and lock numbers and
conditions of the isolation.

5.6 RADIATION PROTECTION OFFICER / SUPERVISOR (RPO/RPS)


The person with the technical competency and approved by Atomic Energy Licensing Board in
writing to supervise the implementation of the radiation protection program including all the
appropriate radiation protection regulations, measures and procedures.
The RPO shall be responsible to ensure that devices containing radioactive sources shall be
isolated / shuttered or removed as part of preparation for confined space work or work that may
expose people to the radioactive source. He shall complete the Radiation Certificate with all
relevant details.

6. TRAINING AND COMPETENCY

All personnel responsible for performing energy isolation activities shall attend training, which
should cover the following as a minimum:
I. Legislation requirements;
II. Types of energy sources and their hazards;
III. Lessons learned from energy isolation incidents;
IV. Permit to work (PTW) procedure;
V. Energy isolation methods;
VI. LOTO & LOTO process;
VII. PPE for energy isolation;
VIII. Roles key personnel in energy isolation.

Authorized personnel e.g. PTW AA, ACP, AP, RPO should be appointed and certified according
to their respective processes.
The Energy Isolation Training will be included as part of PTW Training. An assessment on the
understanding of energy isolation requirements shall be conducted after the training to certify the
candidates capable of conducting energy isolation within his/her department. The assessment
shall be separated from the PTW assessment.

7. PLANNING & RISK ASSESSMENT

The following should be considered when planning for energy isolation:


1. Plan and organize the work properly taking account of different types of energy sources
and their potential hazards
2. Establish a procedure or JMS and communicate to the workers.
A job safety analysis (JSA) shall be conducted for all energy isolation activities (including
activities not requiring PTW), unless a standard procedure for the job with the necessary
precautions and mitigation incorporated has already been established and workers have been
briefed and trained.
The following should be considered for the JSA:
1. Identify and assess the hazards e.g. Assessment of the failures that can occur during the
isolation activity, the likelihood of these failures, and their consequences, then the
selection of the appropriate type and level of controls
2. The appropriate means to dissipate the energy, including the re-accumulation of stored
energy to a hazardous level
3. The steps to verify that the energy has been isolated effectively in order to prevent injury
or damage to equipment that may result from the release of the stored energy.

8. EQUIPMENT PREPARATION

Before any work commences on plant equipment, care must be taken to ensure that the
equipment is free of any energy source and or toxic or other hazardous substances. The PTW
Approving Authority shall ensure that some or all of the following steps have been taken to
eliminate or mitigate the hazards to ALARP:
• Depressurising and/or cooling
• Removal of hazardous liquids
• Removal of hazardous vapours or gasses
• Removal of hazardous solids
• Specialised cleaning techniques
• Isolation from other equipment to prevent re-contamination
• Provision of means of access and escape
• Testing to confirm absence of hazard

Any purging on complex system shall be assessed on the risk and documented.

9. PHYSICAL ISOLATION

Physical isolation refers to the mechanical isolation of the equipment and is typically applied for
processes containing chemical or thermal energy sources. The mechanism for isolation
depends on the nature of the work and type of energy, and include:
• Positive isolation
• Double block and bleed
• Single valve isolation
The Physical Isolation Certificate (PIC) shall be issued when physical isolation is carried out.
APPENDIX 1 shows the different methods for physical isolation.

9.1 POSITIVE ISOLATION


Positive isolation may be carried out as follows:
a) Spool Removal - Removal of piping section and bolting blank flanges (rated for full line
design pressure) on to potential live ends.
b) Blind / Spade Isolation - insertion of a blind plate or swinging a spectacle plate, between
bolted flanges. Any such insertion must be rated to the original pipe design specification.

Positive isolation is regarded as the most secure method and must always be considered first
when planning construction / maintenance work.
a) Confined space entry. When positive isolation is not feasible, double block and bleed
isolation may be acceptable, only after a procedure developed by operations specific for
the situation has been deliberated and approved by the Offshore Manager (OM) or
designated person.
b) For hot or cold work on a system containing or connected to other systems containing
combustible or explosive material, positive isolation is required. If it is not feasible, a
thorough risk assessment shall be carried out including consideration for special PPE and
precautions before issuance of PTW.
c) Blinds / spades must be installed on the safe side of the isolation device, which has been
isolated, depressurized and cleared of hazardous materials.

Note: In the cases identified above which include heat exchangers, positive isolation shall be
used on both sides of the exchanger.

9.2 DOUBLE BLOCK & BLEED (DBB)


DBB isolation is the most secure form of valve isolation, provided the valves can maintain a
reliable seal under the particular conditions of service.
• The bleed valve shall be locked open and vented to a safe location, where no back
pressure may accumulate.
• The bleed must be verified to be clear of any blockage
• The function of the bleed may also be achieved by the use of a valve with a functioning
body bleed in the open position. A check must be done to confirm that the body bleed is
on the downstream side.

The use of double block valves without a bleed or a single block with downstream bleed may
reduce the risk of leakage, but does not provide an equivalent standard to DBB without the
application of additional safeguards.

9.3 SINGLE VALVE ISOLATION (SVI)


SVI consists of the closure of a single block valve. Where the security of the isolation can be
compromised by operation of a single valve, positive measures to prevent operation must be
taken.
For hot or cold work on a system connected to other systems containing non-hazardous
material, single valve isolation is acceptable, provided there is no possible release of other
hazards such as high pressure and high temperature, in which case positive isolation or double
block and bleed is the preferred choice. If it is not feasible, a specific procedure developed by
operations is to be established, communicated and implemented to address the hazard.

9.4 LIST OF ISOLATION/BLINDING/SPADE


A standard list of isolation/blinding and spade shall be applied when physical isolation of
equipment or processes is required. It is only valid when it has been approved for use by the AA.
The list shall include the following information of the equipment or process as a minimum:
I. Serial number of the blinding/spade list
II. Schematic drawing showing the positions of all blinding/spade points and isolation valves,
including the sampling points if required. The PEFS/P & ID drawing may be used for this
purpose
III. Name of equipment or process and tag number
IV. P &ID or PEFS reference
V. Flange sizes
VI. Piping class
VII. Numbering of the blind or spade positions
VIII. Name & designation of approving authority
IX. Date of approval
X. Revision number of the blinding/spade list
See APPENDIX 4 for samples of standard isolation/blinding/spade lists.
Any amendments to the standard list is allowed only after it has been verified and approved by
the AA that the amendments will not impair the integrity of the intended physical isolation.
If amendments to the isolation/blinding/spade list and PIC are required after a PTW has been
issued, then the PTW shall be recalled and all work shall stop. All amendments shall be reviewed
and approved by the appropriate AA, before the PTW is revalidated.

9.5 OTHER ISOLATION REQUIREMENTS


9.5.1 ELECTRICAL ISOLATION
• A PTW is issued for work on an equipment while electrically driven equipment are
electrically isolated, with the use and issuing of EIC.

• Electrical isolation performed by the ACP/CHARGEMAN is done in the presence of an


operations personnel, ensuring the correct equipment is being isolated.

9.5.2 INSTRUMENTATION
For isolation of secondary instrumentation e.g. transmitters, impulse lines, pressure gauges, the
following steps shall apply:
• The AAR shall carry out the primary isolation and apply LOTO accordingly. For services
below 40 barg, single valve isolation may be applied. For services at or above 40 barg,
double block and bleed shall be applied.
• The RA shall isolate the instrument equipment to be worked on. Where impulse lines have
been opened or instrumentation removed, the open ends shall be closed with a rated
plug.
The above requirements do not apply to isolation primary (process) instrumentation e.g. control
valves, flow meter, orifice plates, for which physical isolation as per Section 5.0 above shall
apply.

9.5.3 OTHER ENERGY SOURCES


Appropriate controls should also be identified and applied in activities containing other energy
sources such as kinetic and potential energy. Typically, they are isolated by mechanical means
e.g.:
• Release of the spring before working on the associated spring controlled equipment
(potential energy)
• Lashing/securing cargo on a barge to prevent dangerous movements during sea swells

10. LOTO IMPLEMENTATION

10.1 LOTO DEVICES


10.1.1 LOCK OUT DEVICES
a) The correct lock out devices shall be used to hold energy isolating devices in the safe
position to prevent inadvertent re-energizing of a machine equipment or piping. Examples
of lock out devices include clamp and lock.
b) For all isolation, the lock-out devices shall have individual numbers that will be used for
recording on the respective certificates and the key management Logbook.

10.1.2 TAGOUT DEVICES


a) Tag is a prominent warning device to warn other personnel that energy isolation device
and the equipment shall not be removed/operated, until the tag is removed.
b) To ease the recognition of energy isolation, the tags shall be provided as follows:

c) The tag shall be placed on isolation devices and the tag specifications are as follows:
• Made of a semi-rigid durable plastic material
• The tag used for the lock out box shall be made of a rip resistant disposal material
with section where name, equipment identification, date and other information may
be written
• Green tag for spade/blind shall be made of a rip resistant disposal material and
shall have a perforated section with a duplicated running numbers which may be
torn off to confirm correct tag installation.

d) For all isolation, the tag shall have individual numbers that will be used for recording on the
respective certificates and they key management Logbook.

10.1.3 LOCKOUT BOX


Lockout Box is a secure box capable of holding keys for LOTO device. The specification for the
Lockout Box shall meet the following:
• Transparent cover
• Weather resistance
• Made of durable material
• Totally enclosed

10.2 ENERGY ISOLATION PROCESS


Refer Appendix 1 for the energy isolation process.

10.3 PHYSICAL ISOLATION CERTIFICATE


The physical isolation certificate (PIC) shall be issued when physical isolation has been carried
out. The PIC should make reference to the blinding/spade list which shows details of the
isolation and record details of all LOTO as and when they are installed and removed.

10.4 ELECTRICAL ISOLATION


Electrical isolation is carried out by the authorised competent person (ACP) or the appointed
person (AP), together with the AA or AAR in the substation, and the RA or RAR if required. The
electrical isolation certificate (EIC) is issued when the isolation has been safely completed with
the corresponding LOTO.

10.4.1 CLEAR & TRY


After all switches, valves, and devices are locked-out and tagged, and the equipment is cleared
of personnel, the RA should verify the effectiveness of the lockout by testing the operation of the
ON/OFF switch, actuators, etc., to confirm that the equipment has now been safely locked out
and the equipment will not start.

10.5 RADIATION CERTIFICATE


The radiation certificate is issued when the ionising radiation source has been safely isolated/
shuttered, locked and tagged or removed.

11. EQUIPMENT PREPARATION FOR ENERGY ISOLATION

Before any work commences, the equipment shall be free of any energy source and hazardous
material. The PTW AA shall ensure that steps are taken to eliminate or mitigate the hazards.
Some of the methods to prepare the equipment include, but not limited to:

a. Depressurising
b. Draining
c. Cooling
d. Purging with nitrogen
e. Flushing with water/steam condensate
f. Purging with low pressure steam
g. Removal of hazardous liquids
h. Removal of hazardous vapours or gases
i. Removal of hazardous solids
j. Specialised cleaning techniques
k. Isolation from other equipment to prevent re-contamination
A detailed procedure shall be developed for preparation of equipment or systems for energy
isolation. The procedure should also address the following:
• Risk of trapped pressure, gas and liquids at low points and dead ends
• Include steps and qualitative measurements to confirm absence of the hazard e.g. sampling
points and methods to conduct a gas test to indicate the absence of flammable vapours, or
the suitability of the atmosphere for breathing.

12. WORK EXECUTION

A separate PTW shall be used to carry out physical isolation of the equipment or process. This
PTW shall be closed out when all isolation has been safely completed and LOTO applied, and
the relevant certificates issued. The PTW for the subsequent work is issued only when the PTW
for the isolation is closed.

The RA shall install his lock on the multiple locking device of the lock-out box, to ensure that the
isolating devices remain disabled. When there are several PTWs issued under the same
isolation, the respective RAs will install their locks on the multiple locking device of the lock- out
box.
The RA (1st RA to start work) shall also verify at site the physical isolation according to
blinding/spade list by removing the perforated section of the spade identification tags and
signing on it. The removed section of the tags shall be attached to the PIC.

13. HANDING OVER

For jobs that extend beyond the current shift or work team, the RA or AA should handover the
keys for any lock-out device to the RA or AA taking over the job. The receiving RA or AA is
responsible to verify the isolation status as indicated in the PIC and EIC.

14. EQUIPMENT TESTING

Equipment testing includes test run and /or troubleshooting of equipment which are still isolated
and there are ongoing PTWs which have not been closed. It is defined as allowable work on
equipment to check or test conditions that cannot be checked or tested when the equipment is
isolated and locked out.
Single point of responsibility shall be assigned and given control of isolation devices such as
related switches, breakers, circuits, and valves. This may require the co-ordination with
personnel from the different disciplines to operate the equipment being tested.
Refer Appendix 2 for equipment testing flowchart.

15. REINSTATEMENT & DE-ISOLATION

When the work is completed and all PTWs are closed out, the respective RAs shall remove their
locks from the lock-out box. The AA’s lock shall remain on the lock-out box.
If further work requiring the same electrical isolation and physical isolation is anticipated, the EIC
and PIC may remain open. After 7 days, weekly revalidation is required.
A separate PTW shall be issued for removal of the physical isolation.
The spade tags are removed and the PTW closed out when it is confirmed that the physical
isolation have been removed.
The AA will then unlock the lock-out box and issue the keys to the respective RAs, who will
proceed to de-isolate all other physical and electrical isolation, and update the EIC and PIC
accordingly.
The EIC and PIC are closed out when all physical isolation have been re-instated and
normalised.
