ENGINEERING, PROCUREMENT, CONSTRUCTION, INSTALLATION AND
COMMISSIONING (EPCIC)
1. GENERAL
This procedure covers safe work practices and steps to safeguard employees and equipment in
a place of work from the hazards of energy sources during offshore campaign, operational and
maintenance activities.
It requires, in part, turns off and disconnects the machinery or equipment from its energy
source(s) before performing testing, commissioning, service or maintenance by lock and tag the
energy-isolating device(s) to prevent the release of hazardous energy and take steps to verify
that the energy has been isolated effectively.
This document will be supported by JSC Health, Safety and Environment documents which
where applicable.
2. SCOPE
This procedure are applicable to any work situation where energy sources may pose a hazard to
workers, equipment and or the environment. Energy sources may include but are not limited to
the following: electrical, mechanical, hydraulic, thermal, chemical, pneumatic, ionizing radiation
etc.

TERM DEFINITION
Approving Authority (AA) : The person authorised under PTW system to approve
and issue PTW in the area concerned. Work involving
increased or unusual hazards such as work involving
open flame, hot tapping or entry into a confined space,
shall require review and authorization at a higher level. As
such the AA shall ensure that any equipment being
worked on shall be isolated and made safe of any energy
hazard.
Approving Authority : The person authorised by the Approving Authority (AA) to
Representative (AAR) carry out safety precautions prescribed in the
permits/certificates prior to commencement of work. This
person must also be qualified and whose job requires
him/her to operate or use a machine or equipment on
which servicing or maintenance is being performed under
lockout or tag out, or whose job requires him/her to work

• Disconnection of piping
• Removed valves
• Removed spool piece
• Removed spacer or expansion joint in the piping leading to
the equipment or system to be worked on
• Installation of a blank flange or cap on one or both of the
open ends.
• Insertion of spades or plates capable of withstanding the
maximum design pressure or swinging closed a full pressure
spectacle blind in the piping between the equipment and the
rest of the plant or the source of hazardous material. In
certain cases where the on spec blind/spades cannot be
inserted, off spec blinds/spades may be used for positive
isolation. A risk assessment must be performed before off
spec blinds/spades are used.
Isolation of Ionizing : Ionizing radiation sources such as nucleonic level
Radiation detectors may exist in some PETRONAS facilities. These
radioactive sources must be isolated, shuttered or
removed as part of preparation for confined space work or
work that may expose workers to ionizing radiation.
Radiation Protection : An individual duly approved by the Atomic Energy
Officer/Supervisor Licensing Board and authorized in writing by the
(RPO/RPS) contractor or OPU management. He shall also be the
competent person of the contractor or OPU.
Receiving Authority (RA) : A person authorized by PFLNG2 to receive approved
permits and shall be responsible for supervising the
execution of the work by Work Leaders as per the
precautions in the permit. The Receiving Authority (RA)
can also be the Work Leader (WL). The RA shall specify
controls for safe work and requirements of other
certificates. The RA shall also be responsible to ensure
that the equipment being worked on is correctly isolated,
and the lock out and tag out has been completed before
the commencement of servicing or maintenance on the
equipment.
Receiving Authority : The person authorised by the Receiving Authority (RA) to
Representative (RAR) ensure all the safety precautions prescribed in the
permits/certificates are carried out prior to
commencement of work.
Construction and/or : Workplace activities such as constructing, installing, setting
maintenance up, adjusting, inspecting, modifying, and maintaining and/or
servicing machines or equipment. These activities include
lubricating, cleaning or unjamming machines or equipment
and making adjustments or tool changes where the
employee may be exposed to the unexpected energization
or startup of the equipment or release of hazardous energy.
Setting up : Any work performed to prepare a machine or equipment
to perform its normal production operation.

5. RESPONSIBILITY
supervision of the Approving Authority. The AAR shall be competent to operate the equipment
and understand the process system being isolated.
All personnel responsible for performing energy isolation activities shall attend training, which
should cover the following as a minimum:
I. Legislation requirements;
II. Types of energy sources and their hazards;
III. Lessons learned from energy isolation incidents;
IV. Permit to work (PTW) procedure;
V. Energy isolation methods;
VI. LOTO & LOTO process;
VII. PPE for energy isolation;
VIII. Roles key personnel in energy isolation.

Authorized personnel e.g. PTW AA, ACP, AP, RPO should be appointed and certified according
to their respective processes.
The Energy Isolation Training will be included as part of PTW Training. An assessment on the
understanding of energy isolation requirements shall be conducted after the training to certify the
candidates capable of conducting energy isolation within his/her department. The assessment
shall be separated from the PTW assessment.
8. EQUIPMENT PREPARATION
Before any work commences on plant equipment, care must be taken to ensure that the
equipment is free of any energy source and or toxic or other hazardous substances. The PTW
Approving Authority shall ensure that some or all of the following steps have been taken to
eliminate or mitigate the hazards to ALARP:
• Depressurising and/or cooling
• Removal of hazardous liquids
• Removal of hazardous vapours or gasses
• Removal of hazardous solids
• Specialised cleaning techniques
• Isolation from other equipment to prevent re-contamination
• Provision of means of access and escape
• Testing to confirm absence of hazard
Any purging on complex system shall be assessed on the risk and documented.

9. PHYSICAL ISOLATION
Physical isolation refers to the mechanical isolation of the equipment and is typically applied for
processes containing chemical or thermal energy sources. The mechanism for isolation
depends on the nature of the work and type of energy, and include:
• Positive isolation
• Double block and bleed
• Single valve isolation
The Physical Isolation Certificate (PIC) shall be issued when physical isolation is carried out.
APPENDIX 1 shows the different methods for physical isolation.
Positive isolation is regarded as the most secure method and must always be considered first
when planning construction / maintenance work.
a) Confined space entry. When positive isolation is not feasible, double block and bleed
isolation may be acceptable, only after a procedure developed by operations specific for
the situation has been deliberated and approved by the Offshore Manager (OM) or
designated person.
b) For hot or cold work on a system containing or connected to other systems containing
combustible or explosive material, positive isolation is required. If it is not feasible, a
thorough risk assessment shall be carried out including consideration for special PPE and
precautions before issuance of PTW.
c) Blinds / spades must be installed on the safe side of the isolation device, which has been
isolated, depressurized and cleared of hazardous materials.
Note: In the cases identified above which include heat exchangers, positive isolation shall be
used on both sides of the exchanger.
The use of double block valves without a bleed or a single block with downstream bleed may
reduce the risk of leakage, but does not provide an equivalent standard to DBB without the
application of additional safeguards.
9.5.2 INSTRUMENTATION
For isolation of secondary instrumentation e.g. transmitters, impulse lines, pressure gauges, the
following steps shall apply:
• The AAR shall carry out the primary isolation and apply LOTO accordingly. For services
below 40 barg, single valve isolation may be applied. For services at or above 40 barg,
double block and bleed shall be applied.
• The RA shall isolate the instrument equipment to be worked on. Where impulse lines have
been opened or instrumentation removed, the open ends shall be closed with a rated
plug.
The above requirements do not apply to isolation primary (process) instrumentation e.g. control
valves, flow meter, orifice plates, for which physical isolation as per Section 5.0 above shall
apply.
c) The tag shall be placed on isolation devices and the tag specifications are as follows:
• Made of a semi-rigid durable plastic material
• The tag used for the lock out box shall be made of a rip resistant disposal material
with section where name, equipment identification, date and other information may
be written
• Green tag for spade/blind shall be made of a rip resistant disposal material and
shall have a perforated section with a duplicated running numbers which may be
torn off to confirm correct tag installation.
d) For all isolation, the tag shall have individual numbers that will be used for recording on the
respective certificates and they key management Logbook.
Before any work commences, the equipment shall be free of any energy source and hazardous
material. The PTW AA shall ensure that steps are taken to eliminate or mitigate the hazards.
Some of the methods to prepare the equipment include, but not limited to:
a. Depressurising
b. Draining
c. Cooling
d. Purging with nitrogen
e. Flushing with water/steam condensate
f. Purging with low pressure steam
g. Removal of hazardous liquids
h. Removal of hazardous vapours or gases
i. Removal of hazardous solids
j. Specialised cleaning techniques
k. Isolation from other equipment to prevent re-contamination
A detailed procedure shall be developed for preparation of equipment or systems for energy
isolation. The procedure should also address the following:
• Risk of trapped pressure, gas and liquids at low points and dead ends
• Include steps and qualitative measurements to confirm absence of the hazard e.g. sampling
points and methods to conduct a gas test to indicate the absence of flammable vapours, or
the suitability of the atmosphere for breathing.
A separate PTW shall be used to carry out physical isolation of the equipment or process. This
PTW shall be closed out when all isolation has been safely completed and LOTO applied, and
the relevant certificates issued. The PTW for the subsequent work is issued only when the PTW
for the isolation is closed.

The RA shall install his lock on the multiple locking device of the lock-out box, to ensure that the
isolating devices remain disabled. When there are several PTWs issued under the same
isolation, the respective RAs will install their locks on the multiple locking device of the lock- out
box.
The RA (1st RA to start work) shall also verify at site the physical isolation according to
blinding/spade list by removing the perforated section of the spade identification tags and
signing on it. The removed section of the tags shall be attached to the PIC.
For jobs that extend beyond the current shift or work team, the RA or AA should handover the
keys for any lock-out device to the RA or AA taking over the job. The receiving RA or AA is
responsible to verify the isolation status as indicated in the PIC and EIC.
Equipment testing includes test run and /or troubleshooting of equipment which are still isolated
and there are ongoing PTWs which have not been closed. It is defined as allowable work on
equipment to check or test conditions that cannot be checked or tested when the equipment is
isolated and locked out.
Single point of responsibility shall be assigned and given control of isolation devices such as
related switches, breakers, circuits, and valves. This may require the co-ordination with
personnel from the different disciplines to operate the equipment being tested.

When the work is completed and all PTWs are closed out, the respective RAs shall remove their
locks from the lock-out box. The AA’s lock shall remain on the lock-out box.
If further work requiring the same electrical isolation and physical isolation is anticipated, the EIC
and PIC may remain open. After 7 days, weekly revalidation is required.
A separate PTW shall be issued for removal of the physical isolation.
The spade tags are removed and the PTW closed out when it is confirmed that the physical
isolation have been removed.
The AA will then unlock the lock-out box and issue the keys to the respective RAs, who will
proceed to de-isolate all other physical and electrical isolation, and update the EIC and PIC
accordingly.
The EIC and PIC are closed out when all physical isolation have been re-instated and
normalised.
