This document describes the e-Form process for the Pradhan Mantri Gram Sadak Yojana online management system. It consists of 3 parts: 1. PIU login process - Project Implementation Units download inspection reports, fill them offline in Adobe Acrobat, and upload the completed forms for review. Errors are displayed and must be corrected. 2. QM login process - Quality Monitors follow the same process to download, fill offline, upload, and correct errors on their inspection reports. 3. The system prefills available data and has validation checks to verify information. Prerequisites are the Adobe Acrobat Reader and internet access to download and upload forms.

About e-Form
• A new module to upload digitized inspection reports by PIU and
Quality Monitors (NQM/SQM)

• Data entry is in Offline mode

• PART-I and PART-II to be downloaded from OMMAS by PIU and Quality


Monitors resp.

• In downloaded PDF, information already available in OMMAS will be in


prefilled format and non editable.

• Validation Checks are built to verify the data entered in PDF

• Uploading of PDF through OMMAS

• PIU to select month, year & e-Form status for which Inspection report is
to be filled & click “Get List”.
