I. Bronchoscopy: 15points
Mr. Hinga is scheduled for a bronchoscopy for the diagnostic purpose of locating a
pathologic process.
1. State your instructions about the procedure to Mr. Hinga. 5points
-Good day, ma’am Hinga I’m Adrea Garino your nurse for today. I just want to
inform you that Bronchoscopy is a procedure wherein there is an instrument inserted either into
your nose to visualize the inside of your airways to detect abnormalities for me and the physician
to give proper interventions about your condition. Bronchoscopy is performed by a physician or
pulmonologists.

2. What nursing measures you would explain before the procedure? 5points
- I will explain and instruct the patient not to eat or drink for four to eight hours
before procedure to prevent obstruction of the instrument and to clearly visualize the airways. I
will also explain to the patient to stop taking blood thinner medications to decreases the risk of
bleeding during the procedure. Furthermore, I will ask the patient to remove dentures, hearing
aids, and glasses if present to avoid inconvenience while during the procedure.

2. State at least 5 nursing responsibilities after bronchoscopy.


- Assess bleeding tendencies. Observe the patient’s sputum and report any
excessive bleeding. Explain that minimal amount of blood streak is expected and
normal for few hours after procedure.
- Assess respiratory status. Monitor the respiratory status for any signs of
hypoxemia, hemorrhage, and chest tightness
- Position the patient. Place the conscious patient in a semi-fowler’s position
while for an unconscious patient, place on the side with the head of the bed
slightly elevated.
- Prevent aspiration. Provide an emesis basin and instruct the patient to spit out
saliva rather than swallow it.
- Relieve anxiety and provide comfort measures. Reassure the patient that
hoarseness, loss of voice and sore throat may occur temporarily.

II. Thoracentesis: 10points:


Mrs. Lima is admitted to the clinical area for a thoracentesis. The physician wants to
remove excess air from the pleural cavity.
1. State and elaborate five (5) nursing responsibilities before the thoracentesis. 5points
- Asked the patient to sign a consent form that gives permission to do the procedure.
- Vital Signs (heart rate, blood pressure, respiratory rate, and oxygen level) are to be
assess before the procedure.
- Asked the patient to remove any clothing, jewelry, or other objects that may interfere
with the procedure.
- The patient may receive a sedative prior to the procedure to help the patient relax.
-Inform the client not to cough while the needle is inserted to avoid puncturing the lung.
2. Identify five (5) observations after thoracentesis that are essential for deliberation of
patient care. 5points.
- Redness or swelling of the needle site
- Blood or other fluid leaking from the needle site
- Feeling of shortness of breath
- Trouble breathing
- Chest pain

III. Thoracoscopy: 25points


Your patient is scheduled for a video-assisted thoracoscopy.
1. What is thoracoscopy? 5points
- Thoracoscopy is a procedure to look inside the chest cavity with an endoscope. An
endoscope is a narrow bendable tube that has a light and camera on the end. The
endoscope is inserted into the chest through small incision in the chest wall.
Thoracoscopy may be done to diagnose or treat conditions of the lungs and pleura (thin
lining covering the lungs).

2. Describe the post procedure nursing care and teaching for a patient undergoing this
procedure. Identify the specific assessment parameters that are indicated. 20points.
- Teach the patient or the significant others on how to clean the wound site so that
after discharge they can do it independently.
- Pain Management. Give the patient pain killers if needed
- Respiratory management. It is important to monitor the patient for risk of
difficulty breathing
- Chest tube care. Mae sure water is bubbling gently. If not quickly and carefully
reposition patient to see if blockage is the reason. If this does not fix the problem
this is an emergency and lead to excessive pressure in the chest.
- Have at the bedside always sterile occlusive gauze and padded clamps incase tube
becomes dislodged. Be very careful when milking tube, it can cause more damage
than good.
