Good day!

The undersigned counsel most respectfully furnishes you the Certificate of Appearance dated 26
May 2021 and Billing Statement dated 07 June 2021 in the case of Helen Gayas-Farinas, et al. vs.
Rural Bank of Dao, (Capiz) Inc., now known as Bangko Buena Consolidated Inc. (A Rural Bank) and
Lynette Asumbra vs. Philippine Prudential Life Insurance Company, Civil Case No. M-16-0441-08,
Regional Trial Court of Mambusao, Capiz, Branch 20.1

At the scheduled Pre-Trial Conference, the Presiding Judge was indisposed. It is noted that prior to the
said hearing the Hall of Justice of Mambusao, Capiz was closed due to disinfection and Poblacion,
Mambusao, where the court is located, was placed on COVID lockdown. The Court will send a notice to
the parties for the schedule of the Pre-trial Conference.
