To Whom It May Concern;

I, FILLINE BIEN A. LECIAS, currently a new student who intent to enroll in MA


SPED Specialized in Learning Difficulty hereby authorize JOEL E. MONJARDIN, from
Mangga St. Dumanlas, Buhangin to obtain from Holy Corss Davao College, Inc. the
following documents:

 Transcript of Records/ TOR


 Certificate of Honorable Dismissal (CHD)Certificate of Good Moral
 Other documents needed for transfer in graduate school.

I am not able to represent myself because I am have a work at Lamanan Elementary


School 8:00 am-5:00 pm Monday to Friday. Thus, I needed to transfer to your school for
Graduate School Masteral.

In addition, the Honorable Dismissal received today from RMC will be relayed to the
school, in order to release the other documents with your response in acceptance of my
transfer.

I hereby authorize JOEL E. MONJARDIN to act on my behalf and perform all the


necessary transactions required, to complete the process of FILLINE BIEN A. LECIAS and
shall be presenting his or her Identification card for verification. I also permit my proxy to
receive the above mentioned documents for whatever legal purposes it may serve.

JOEL E. MONJARDIN as my proxy is not allowed to use the received documents for
any other purpose not mentioned in the letter and he/she should not also be allowed to make
any other decisions on my behalf without written consent from the Holy Cross Davao
College.
