ORSOLINO - Process Recording - Termination Phase

Okay, I understand. You've come a long way and should feel proud of your progress. Is there anything else you'd like to discuss before we conclude our session today? Patient: "Wala na po, salamat sa pag-alala sa akin nurse. Nakakatuwa na may tumutulong sa akin." Nurse: You're very welcome. I'm glad we could talk and that I could provide some support. Please don't hesitate to reach out if you need anything else in the future. You've got this! Wishing you all the best moving forward. Patient: "Salamat po ulit. Ingat kayo pauwi." Nurse: Thank you, you

Respectfully Submitted to

Alma Trinidad Taragua

FEBRUARY 13, 2023

Name of the Patient: Nina Rica Diongzon

Name of the Nurse: Erykah Gheil Orsolino

Observation prior to the interaction:

The nurse-patient interaction began on March 10, 2023, at about 8:00 am and lasted for
approximately one hour before coming to an end. Before the interaction, the patient is already
done with breakfast. She possesses a medium build, short hair, black eyes, fair skin, symmetrical
features, and wears corrective glasses. The patient is an adult female who is 30 years old. She is
well-groomed and free of offensive odors. During the course of the interview, she maintained a
cheerful demeanor, which gave the impression that she was enjoying herself. Since the patient is
now familiar with the nurse, the two of them can begin to establish a nurse-patient relationship.
The atmosphere was soothing and quiet. The living room was spotless and completely free from
distractions. The room remained comfortably cool and pleasant due to the electric fans. The
room is well lit and ventilated.

The client states that
during the first time she
tried to quit smoking, she
experienced severe
withdrawal symptoms
such as agitation and a
lack of sleep; however,
she persisted in her
efforts to quit smoking
despite these difficulties.
In addition to this, she

Seeking Clarification:
The process of eliciting
clarification involves
asking the client to
provide more specific
details about the topic
they provide. If a
customer says something
that isn't clear, it's crucial
that you ask them to
elaborate. The nurse
inquired about how the
client was feeling and
what the client meant
when she talked about her
aura and how she felt it
was radiating.

affirmations like “I see,

wants to convey.

General Leads:
This is a follow-up
comment that encourages
the patient to continue
talking and expand on the
topic that they have been
discussing. The patient
responded to the student
nurse's question about
what is going on in her
life right now by stating
that in order to keep from
thinking about smoking,
she distracts herself with
a variety of different
coping mechanisms.
