Informed Consent Form for Level of Satisfaction on The Quality Dimensions of


Learning Management System among Junior High School Students in Starchild
Innovative Learning Academy

INTRODUCTION

You are invited to participate in a research study conducted by Master of Arts in

Educational Management students of Davao del Norte State College because you fit the

inclusion criteria as respondents of our study. Your involvement is entirely voluntary.

Before deciding whether to participate, please read the information below, and ask

questions about something you do not understand. Please take as much time to read

the consent form as you like. You can also decide to talk with your family or friends

about involvement. You will be asked to sign this form if you wish to participate. A copy

of this form will be given to you.

PURPOSE OF THE STUDY

This study aims to determine the level of satisfaction of the Junior High School students

with the utilization of the learning management system particularly the Google

Classroom at the Starchild Innovative Learning Academy, specifically: (1) To determine

the demographic profile of the participants of the study in terms of; Gender, Age Group,

Year level and Available technology (2)To find out if there is a significant difference in

the level of student's satisfaction in utilizing Learning Management System in terms of;

Gender, Age Group, Year level, Available technology (3) To find out if there is a there a
significant relationship between the quality dimensions of learning management system

in the students’ level of satisfaction in terms of; System Quality, Information Quality and

Service Quality.

STUDY PROCEDURES

If you volunteer to participate in this study, you will be asked to participate by answering

the online survey questionnaire which you can finish in less than 30 minutes.

POTENTIAL RISKS AND DISCOMFORTS

As a participant in the study, you have the option of choosing not to respond to the

survey questions if you are uncomfortable with the possibility that the study procedure

and survey questionnaire will interfere with your normal activities, or you can choose to

withdraw if you are unable to provide the information that was requested. Your

cooperation is valued by the researchers, and they will prioritize your wellbeing during

the study.

POTENTIAL BENEFITS TO PARTICIPANTS AND/OR TO SOCIETY

The results of this study may produce pertinent data that parents and school

administrators of the students may find beneficial. The findings, debates, and outcomes

of this study can stimulate the development of factual knowledge that can be utilized to

enlighten students about the advantages of being aware of a learning management

system's quality dimensions.

CONFIDENTIALITY

As far as the law allows, we shall maintain the confidentiality of your study-related

records. Any personally identifiable data collected in conjunction with this study will be
kept private, unless it becomes necessary to do so in order to safeguard your rights or

welfare. With this certificate, the researcher has the right to withhold information about

your participation from those not involved in the study. There will not be any use of

personally identifying information when the research results are published or presented

at conferences.

PARTICIPATION AND WITHDRAWAL

You have the option to participate. No penalties or advantages to which you are

otherwise entitled will be lost because of your unwillingness to participate. You are free

to stop participating at any moment and revoke your consent without incurring any fees.

By participating in this research study, you are not surrendering any claims, rights, or

recourse you may have.
