SAFE WORK METHOD STATEMENT –

Combined SWMS for High Risk Activities

NOTE: All high-risk construction works must have a SWMS prepared. Work must be performed in accordance with this SWMS.

This SWMS must be kept and be available for inspection until the high-risk construction work to which this SWMS relates is completed. If the SWMS is revised, all versions should be kept. If a notifiable
incident occurs in relation to the high-risk construction work in this SWMS, the SWMS must be reviewed and revised if needed and kept for at least 2 years from the date of the notifiable incident.

 If a stop work is called, the work team must review the SWMS and either modify the existing controls or introduce new controls, so the work can be completed safely. Work should not
recommence until these controls have been documented in the SWMS and implemented.

Following a WHS incident at an Optus workplace, which is notifiable to a regulator, please follow these steps:

1. Call 000 if required, render first aid to any injured workers, and/or ensure the environment is free from immediate danger;

2. Preserve the scene of the incident.

3. Immediately contact the Optus Serious Incident Reporting Hotline on 02 8328 0707, from the scene of the incident where possible, and advise that you have had a notifiable incident in an
Optus workplace. Alternatively, you can report the incident to your Optus Networks HSE Manager. It is important to ensure you speak with an Optus HSE Manager as a priority, either
directly, or via the hotline.

4. Follow instructions from Optus HSE before the site /scene can be disturbed,
