You are invited to participate in a research study conducted by Kierin Caitlin Go, Hansel Mari Vic Hugo
and Mohamad Iskak, at the University of the Immaculate Conception, because you fit the inclusion
criteria for informants of our study.

Your participation is completely voluntary. Please read the information below, and ask questions
about anything you do not understand, before deciding whether to participate. Please take as much
time as you need to read the consent form. You may also decide to discuss participation with your
family or friends. If you decide to participate, you will be asked to sign this form. You will be given a
copy of this form.

nahiangay ka sa mga sukaranan sa paglakip alang sa mga impormante sa among pagtuon.

Ang imong pag-apil hingpit nga boluntaryo. Palihug basaha ang kasayuran sa ubus, ug pangutan-a
ang mga pangutana bahin sa bisan unsang butang nga wala nimo masabut, sa wala pa paghukum

PURPOSE OF THE STUDY

The study aims to examine the experiences, knowledge, and motivations of patient-consumers from
urban poor areas regarding irrational antimicrobial use. The results of this study may contribute to
the development of new policies and protocols in purchasing antimicrobial drugs in Digos City and
possibly on a larger scale. The results of the study will provide a better understanding of the
experiences, knowledge, and motivation of consumers regarding antibiotic use as well as the
interactions between these factors. This information can be used as a basis to improve antibiotic
dispensing and distribution protocols as well as information dissemination regarding antibiotic
resistance. Doing so may lead to increased patient awareness and adherence to antibiotic use, which
may help control the spread of antibiotic resistance. Moreover, the information that can be yielded
from this study, excluding private information from participants, can be shared with the University of
the Immaculate Conception College of Pharmacy and Chemistry. Such actions are valuable when
information from this study is relevant to activities in the college, such as seminars or class
discussions. Information can also be shared with pharmacists or other professionals who find this
study relevant to their profession. Lastly, this study may be valuable to researchers who wish to study
antibiotic use and misuse in the Philippines.

resistensya sa antibiotiko. Ang paghimo niini mahimo nga hinungdan sa pagdugang sa pagkaamgo sa
pasyente ug pagsunod sa paggamit sa antibiotiko, nga mahimong makatabang sa pagkontrol sa
pagkaylap sa resistensya sa antibiotiko. Dugang pa, ang kasayuran nga mahimo’g makuha gikan sa
kini nga pagtuon, wala’y labot ang pribado nga kasayuran gikan sa mga partisipante, mahimong
ipaambit sa University of the Immaculate Conception College of Pharmacy and Chemistry. Ang ingon
nga mga lihok hinungdanon kung ang kasayuran gikan sa kini nga pagtuon adunay kalabotan sa mga
kalihokan sa kolehiyo, sama sa mga seminar o diskusyon sa klase. Mahimo usab ipaagi ang
kasayuran sa mga parmasyutiko o ubang mga propesyonal nga nakit-an nga kini nga pagtuon nga
adunay kalabutan sa ilang propesyon. Sa katapusan, kini nga pagtuon mahimong hinungdanon sa

Pilipinas

STUDY PROCEDURES
Before participating in this study, you will get an invitation to participate from the researchers, as well
as Ethics Informed Consent Forms for the focused group discussion and in-depth interview. The
researchers will then notify the participants of the discussion's schedule and send the link to them via
your personal e-mail address. Link for an in-depth interview will be sent through email as well. We
will extract relevant details and information from you and other respondents simultaneously identified
to undergo such group discussion. The discussion will be conducted online as an in-depth interview
via Google meet to meet the satisfaction for the collection of qualitative data and with the initiative of
a feasible, proper platform due to the restrictions and risks set by the current pandemic. Upon
entering the Google meet platform, an orientation will be given for 30 minutes to know what to know,
what to do and what to expect during the discussion by the facilitators. The facilitator will then
proceed to the actual data gathering procedure within 1-2 hours depending on the value of the
discussion. Lastly, post-conferences will be initiated for any additional discussion, clarifications, or
questions you have towards the researchers and the study in general within 30 minutes. As for the
in-depth interview, the collection of the data was done as soon as the participants answered the
Google form. You will be guaranteed anonymity by assigning code names and assured confidentiality
that data collected will be used for mentioned research purposes intended for this study only.

sa iskedyul sa diskusyon ug ipadala ang link sa kanila pinaagi sa imong kaugalingon nga e-mail

namon ang may kalabutan nga mga detalye ug kasayuran gikan kanimo ug uban pang mga

katapusan, ang mga komperensya nga post-gipasiugdahan alang sa bisan unsang dugang nga
diskusyon, pagklaro o mga pangutana nga adunay ka sa mga tigdukiduki ug sa pagtuon sa
kinatibuk-an sa sulud sa 30 minuto. Sama sa alang sa halawom nga pakigsulti, ang pagkolekta sa
datos nahimo dayon sa pagtubag sa mga sumasalmot sa porma sa Google. Gagarantiyahan ka nga
dili magpaila sa ngalan pinaagi sa pagtudlo sa mga ngalan sa code ug masiguro ang
pagkakumpidensyal nga ang datos nga nakolekta magamit alang sa gihisgutan nga mga katuyoan sa
panukiduki nga gituyo alang sa kini nga pagtuon.

POTENTIAL RISKS AND DISCOMFORTS

Due to the health protocols, restrictions, threats and risks set by the current COVID 19 pandemic,
The participants will not be exposed to any physical risk for this study will be strictly conducted
through online means. Strictly using online means also ensures the safety of participants, in
compliance with IATF guidelines. To prevent any psychological risks. Moreover, the anonymity of the
participants will be assured, as well as the confidentiality of the data they will be providing. Since this
study involves in-depth interviews that dive into the experiences, knowledge, and motivations of the
participants, they may be uncomfortable in sharing their thoughts. To remedy this, the researchers
will provide aliases or codenames to ensure anonymity and a non-disclosure agreement will also be
employed to ensure the confidentiality of their responses. After the data gathering procedure,
participants will be given the contact information of the researchers so that, if they wished, they can
withdraw their participation at a later date and also ask for a copy of the study once it is approved.
This ensured that the rights of the participants to privacy will be respected, and the research will be
accessible to the community.

POTENTIAL BENEFITS TO PARTICIPANTS AND/OR TO SOCIETY
Consumer experiences, knowledge, and motivation regarding antibiotic usage, as well as the
connections between these elements, will be better understood as a result of the study's findings.
This data can be utilized to improve antibiotic dispensing and distribution procedures, as well as
antibiotic resistance information dissemination. As a result, patient knowledge and adherence to
antibiotic use may improve, potentially reducing antibiotic resistance transmission.

Ang mga kasinatian sa nahibal-an, nahibal-an, ug panukmod bahin sa paggamit sa antibiotiko, ingon
man mga koneksyon taliwala sa kini nga mga elemento, mas masabtan ingon usa ka sangputanan sa
mga nahibal-an sa pagtuon. Ang kini nga datos mahimong magamit aron mapaayo ang mga pamaagi
sa paghatag ug pagbahin sa antibiotiko, ingon man ang pagsabwag sa kasayuran sa resistensya sa
antibiotiko. Ingon usa ka sangputanan, ang kahibalo sa pasyente ug pagsunod sa paggamit sa
antibiotiko mahimong molambo, nga mahimo’g maminusan ang pagbalhin sa resistensya sa
antibiotiko.

DATA PRIVACY AND CONFIDENTIALITY

The rights and dignity of every participant will be respected, especially those involving privacy and
confidentiality. To remedy this, confidentiality will be ensured by providing aliases or code names that
will guarantee anonymity and assure that the Data Privacy Act of 2012 will be observed throughout
the data gathering procedure. The demographic profile, personal information, and all the responses
of the participants will be kept confidential and will only be known to the researchers, facilitators,
advisers, and panelists. Moreover the researchers modified a study from Karuniawati and colleagues
(2020), participants will be assured that only the researchers will have access to the Google Meet
recording and that after transcribing all the data needed it will be deleted to ensure that it will not be
disseminated. Most importantly, participants will be informed of all these rights that are reserved for
them in order to properly practice it.
