CALAMITIES OR EMERGENCIES IN THE MUNICIPAL GOVERNMENT OF DOÑA
REMEDIOS TRINIDAD, BULACAN

MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT made and executed by and between;

The Municipal Government of Doña Remedios Trinidad represented by the Municipal


Mayor HONORABLE RONALDO T. FLORES, referred as the LOCAL GOVERNMENT OF
DRT.

-and-

DAMIAN FUNERAL PARLOR, a funeral institution with crematory services at San


Miguel, Bulacan, represented by its owner, LORENZO DAMIAN referred as DAMIAN;

WITNESSETH

WHEREAS, President Rodrigo Roa Duterte issued Proclamation No. 922 declaring a State
of Public Health Emergency throughout the Philippines on March 8, 2020;

WHEREAS, the President Rodrigo Roa Duterte likewise issued Proclamation No.929
declaring a state of calamity throughout the Philippines due to Corona Virus Disease 2019;

WHEREAS, DILG Memorandum Circular No. 2020-063, dated March 27, 2020 entitled
“Interim Guidelines on the Management of Human Remains for patient Under Investigation (PUI)
and Confirmed Coronavirus Disease (COVID-19) Cases was issued to guide the LGUs in the
Management of the human remains, to wit;

“Section 4.6 LGUs shall designate and commission reputable funeral parlors and
crematorium to handle the remains of confirmed (COVID-19) cases and PUIs, and provide
possible financial assistance to cover the logistics, fuel and salary and other expenses that will be
incurred in the process which include transfer/transport of human remains;”

WHEREAS, the LOCAL GOVERNMENT OF DRT in compliance with the above-


mentioned memorandum will contract out crematorium services to DAMIAN for those deceased
PUI and patients who died positive of COVID 19;

WHEREAS, DAMIAN is the only funeral institution near this municipality which has
crematory services

NOW THEREFORE, in view of the foregoing premises, the parties agree on the following;

A. DAMIAN:
1. To provide crematory services to deceased PUIs and positive COVID-19 patients in the
municipality of DRT, to wit,
a. Processing and cremation - Php 45,000.00
b. Handling and Pick-up - Php 5,000.00
- Additional Php 3,000.00 if the pick up is during night time.
B. THE LOCAL GOVERNMENT OF DRT:
1. To pay the amount of above indicated in cases of referral of deceased PUIs and positive
COVID-19 patients,
IN WITNESS WHEREOF, the parties of this Agreement have on this ___day of _______,
2023 hereunto set their hands at the bottom of this Agreement at Municipality of Doña Remedios
Trinidad, Bulacan.
