JOINT AFFIDAVIT OF COHABITATION

age, single, Filipino citizens, with residence address at
_________________________, Philippines, after being sworn to in accordance
with law do hereby depose and state: THAT –
1. We are both singles and have no legal impediments to
marry each other;
2. We are living together as husband and wife without the
benefit of marriage sometime on __________________, or a
period of more than 6-years at __________________________,
Philippines;
3. Out from the said cohabitation, we begot a child on
_____________________, in the name _____________________, a
copy of the Certificate of Live Birth is hereto attached and
marked as Annex “A”; and
4. To legally legitimize our so called “family”, we have decided
to settle down and will avail the benefit of civil marriage
provided for under Art. 34 of the Family Code of the
Philippines.
We are executing this affidavit to attest the genuineness and the
veracity of the above-mentioned statements and to whatever legal purpose
this may serve.
