WE, ___________, of legal age, married, Filipino, and with residence at


_____________ and ___________, of legal age, Filipino, and with residence at
_______________, after having duly sworn to in accordance with law hereby depose and
state:

1. About 7 o’clock in the evening of 13 July 2023, during heavy rains and while
traversing New Port City, Pasay City, nearby Ninoy Aquino International
Airport (NAIA) Terminal 3, Mr. ________ driving a Toyota Avanza with Plate
No. _______, accidentally bumped to a Mercedes Benz C 180 Avant Gard with
Plate No. ________driven by Mr. ______;
2. The Toyota Avanza suffered dent and scratches, while the Mercedes Benz C 180
suffered paint damage and scratches;
3. Due to heavy rains and traffic inconvenience brought by the incident, the drivers
agreed to enter into a further agreement upon proper assessment of the damages
caused;
4. That upon agreement, Mr. ______ and the registered owner of the Toyota
Avanza, _________, will properly compensate, thru its available Compulsory
Third Party Liability Insurance, the actual damages caused to Mr. _____ vehicle;
5. That both parties agreed that Mr. ______will cause the repairs to the Toyota
Avanza without any compensation from Mr. ________.
6. That the parties agreed, as well, to settle peacefully and amicably and, as far as
practicable to not file any suit—administrative, civil or criminal case against the
party at fault.

That we are executing this Affidavit to attest to the truth of the foregoing.
