This informed consent form has two parts: (1) Information Details (information about

the study and the details of participation), and (2) Certificate of Consent (certification that the

participants were given the information about the study). The informed consent form will be

translated to Filipino version. Each participant will be given copies after the researcher

I am Roy Ivan V. Delos Reyes, a candidate for Master of Science in Criminal Justice with

Specialization in Criminology at De La Salle University – Dasmariñas. This study is a

requirement to complete my master`s thesis. This is to formally seek for your participation to

be a respondent in my study entitled “Coping Strategies and Academic Performance of

Criminology Students in Selected HEI`s in Cavite during the transition from online and

modular learning to hybrid learning” to assess problems encountered during the transition

period from online and modular learning to hybrid learning and determine their coping

strategies to be correlated with their academic performance. This will require your time and

effort to participate in answering my adopted and modified research questionnaire. The

researcher will be present during the data gathering procedure to immediately respond on

PURPOSE OF THE RESEARCH

This research is conducted to determine challenges encountered by the students

during the transition period from online and modular learning to hybrid learning and their

coping strategies to be correlated with their academic performance.

TYPE OF RESEARCH INTERVENTION

An adopted and modified questionnaire will be distributed to the respondents. The

instrument will identify the challenges encountered, coping strategies and the academic

performance of the students during the transition period from online and modular learning to

hybrid learning. The researcher made a set of questions to identify the challenges

encountered and adopted the Coping Strategies Inventory created by David L. Tobin and the

Academic Performance Scale of Christopher McGregory to be further correlated. The

research instrument will be validated by three (3) experts in field of education.

PARTICIPANT SELECTION

The respondents of this study will be composed of fourth year criminology students

from the selected Higher Education Institution in Cavite for the reason that the following

students started their collegiate experience with an online and modular learning and mostly

experienced the transition into hybrid learning.

VOLUNTARY PARTICIPATION

The researcher is asking your permission to include yourself as participants

in this study. The participants will be given survey questionnaires to be answered.

Any information that is obtained in connection with this study and that can identify

with you will remain confidential and will be disclosed only with your permission.

Adequate safeguards will be used to maintain the privacy and confidentiality of all

information obtained. When the results of the study are reported, only group

results will be described, not individual results.

Therefore, participation in this study is voluntary. It is up to you to decide

whether or not to take part in this study. If you decide to take part in this study,

you will be asked to sign a consent form. After signing the consent form, you are

still free to withdraw at any time and without giving a reason.

PROCEDURES

The researcher will utilize a quantitative correlation research design. A survey

questionnaire will be administered face-to-face and will be distributed through google forms

to be answered by the respondents voluntarily. There are four (4) parts in the research

instrument, the first part collects the demographic information of the respondents, the second

identifies the challenges encountered during the transition from online and modular learning

to hybrid learning, the third part gather the different coping strategies of the respondents, and

the last part collects the academic performance of the respondents.

For questionnaire surveys:

Based on the agreement between the researcher and the concerned

institution, the researcher will be present face-to-face to distribute survey

questionnaires which will be sent via google form link to the respondents. After

answering the survey, it will be submitted to the researcher protect the

confidentiality of information and anonymity of the participants.

DURATION

The survey will only take 20 to 30 minutes to complete answering the form. This will

be given during the agreed scheduled between the researcher and the institution ensuring that

no academic hour will be interrupted for the said activity.

RISKS

The survey questionnaire that will be sent focuses on the challenges, coping

strategies, and academic performance of the respondents during the transition

period from online and modular learning to hybrid learning only. There are no

known risks associated with this research. Permissions to conduct surveys will be

obtained first before you are surveyed and may withdraw anytime.

BENEFITS

The research will have no direct benefit as participant of the study.

However, researcher hope that the information obtained from this study could aid

in the design and development of programs and strategies in addressing the

problems in the transition period from online and modular learning, and might as

well help the respondents on what are the effective coping strategies that could

help their academic endeavor.

REIMBURSEMENTS

The research entitles no monetary compensation for participation in this

study. However, the researcher will provide treats for the respondents for the time

and effort given during the participation in the said research.

All information obtained from the study will be coded to protect each

respondent. No names or other identifying information will be used when

discussing or reporting data. Every effort will be made by the researcher to

preserve the anonymity and confidentiality of the participant by assigning code

names/numbers to each participant that will be used on all research documents.

The researcher will safely keep all files and data collected in a secured locked

cabinet in the personal possession of the researcher. Once the data has been fully

analyzed it will be destroyed.

SHARING THE RESULTS

During the research, information from this study will be kept in locked files

that only the researcher and adviser can open. Any personal information that could

reasonably identify you will be removed or changed before files are shared with

other researchers or results are made public. The result of this study may be shared

through research presentation and a copy of the result may be sent to the institution

for program development.

Participation in this study is voluntary. It is up to you to decide whether or

not to take part in this study. Withdrawing from this study will not affect your
