Dear Sir,

Kindly refer to the mentioned subject, this is for kind your information that while returning home on
23th May 2023 in rent a cab (White Coloured Huyndai Eon, Vehicle No: WB05-6516) I have left my
wallet in the cab. I have raised complaint with the service provide In-Drive over phone.

Time of Return: 09:38 Pm on 23rd of May 2023


Place of boarding: Ballygunge AC Market
Place of drop: Q-1, Panchasayar, Income Tax Township, Garia, Kolkata -700094
The driver Mr. Santosh (cell phone no: 99031-87701) is denying the fact that the wallet is in his
cab.

The articles which are lost are mentioned below:


1. Driving License in the name of Rajat Kanti Bhanja Choudhury (DL No: WB0520190009254)
2. Certificate of Registration in the name of Rajat Kanti Bhanja Choudhury (WB20C-0860)
3. Aadhar Card in the name of Rajat Kanti Bhanja Choudhury bearing no 4892 5508 4869
4. Axis Bank Debit Card (in the name of Rajat Kanti Bhanja Choudhury, Bank name: Axis Bank
Ltd, New Garia Branch (A/C No: 011010100433563)
5. Indusind Bank Credit Card- Branch: Dalhousie
6. Metro Smart Card

I have also attached the No-Objection Certificate received.

I would request you to kindly register a General Diary in reference to the above mentioned subject
and oblige.
