INITIAL DATABASE FOR FAMILY NURSING PRACTICE
A. FAMILY STRUCTURE, CHARACTERISTICS AND DYNAMICS
They are only 2 living in the house. The head of the family is Mr. Servillano Rediang. He is 56 years old. Living with him is his only child, Servillano
Rediang Jr., 8 years old. They are currently residing at Macalva Central, Agoo, La Union. The type of family is patriarchal in which the father is the head and ruler
of the family. It is a single parent family because the child’s parents are already separated.
B. SOCIO-ECONOMIC AND CULTURAL CHARACTERISTICS
For the income and expenses in the house, it is being provided y the father who works as an Eco Head in their barangay. He was also the one who is
responsible for managing the money. Mr. Rediang is a high school graduate while his child is currently in Grade 3. Their religious affiliation is Jehovah’s
Witnesses. The family is known as law abiding and participates in Barangay activities, knowing that the father is an Eco Head.
C. HOME AND ENVIRONMENT
Their house is made up of cement and some light materials. It was built beside the farm. The house is simple and not well-organized. It has no room; they
only have one single bed on one corner of their house where they sleep. It is adequate for the size of the family since they are only 2 living together. There are
presence of breeding or resting sites of vectors for diseases such as mosquitoes and flies due to many trees that surrounds their house and open water drainage.
They do not have a refrigerator or any sanitary food storage. When cooking, they make use of firewood in the form of logs and branches from trees. Their source
of water supply is manual pump well. For drinking and cooking, sometimes Mr. Rediang will buy mineral water but due to inadequate income, most of the time he
will just boil water that came from the well. They have their own toilet just beside their house but there is no proper cover and roofing. There is no proper garbage
disposal. They have an open drainage system. They have good neighborhood, they are friendly and harmonious. Houses are not that congested. Communication
means are mobile phones and for transportation, few minutes of walk then they can ride on a tricycle going to the main road.
D. HEALTH STATUS OF EACH FAMILY MEMBER
The whole family do not have any past significant illnesses. Their vital signs are in normal range and does not have any present illnesses. Family eats three
times a day. Sometimes, they don’t practice proper hygiene, they eat without properly washing their hands and they don’t wear any foot wears when going outside
or walking in the vicinity of their house. There is modifiable risk with regards to the father which is drinking 5 or more cups of coffee in a day. Also, Mr. Rediang
states that he suffers from arthritis, mostly in his knees when strained and when it’s old.
E. VALUES, HABITS, PRACTICES ON HEALTH PROMOTION, MAINTENANCE AND DISEASE PREVENTION
With regards to their immunization, as far the father remembers, his child completed his immunization except him, he only finished BCG. The father
exercises in the form of doing household chores such as cooking, laundry, and cleaning inside and outside their house. His child sometimes plays with the other
children. Mr. Rediang usually uses his mobile phone while resting. He sometimes has hard time sleeping because of too much caffeine and hours of using his
phone. His child, on the other hand, has adequate sleep and rest. They use bed net when sleeping to protect themselves from mosquitoes. Mr. Rediang also said
that he used to drink and smoke but gradually, he avoided it until he was able to stop it.
