Eligibility
Who is Eligible?
All full-time associates who have met their initial waiting period are considered eligible associates. Full time is
considered working a minimum of 30 hours or more per week.

Who is an Eligible Dependent?


Eligible dependents under the Atrium Hospitality benefits plan include:

Dependents
z You can purchase Voluntary Dependent Life/AD&D in $5,000 increments up to $100,000 (or 50% of your
voluntary life election) for your spouse and increments of $1,000 to a maximum of $10,000 for your
eligible dependent child. These cannot be enrolled in unless the associate enrolls in Voluntary Life.
z Guaranteed issue for Spouse Life is $25,000. All amounts for Child Life are guaranteed issue.

Please note your Life & Disability coverage will end on your date of termination.

In case of an accident or illness, this benefit pays 60% of your base salary to a set maximum. If you are

Voluntary Accident Coverage


Accident coverage is designed to help meet the out-of-pocket expenses and extra bills which can follow an
accidental injury, whether minor or catastrophic. Indemnity lump sum benefits through Reliance Standard are
paid directly to you based on the amount of coverage listed in the summary of benefits. The accident plan is
guaranteed issue, so no health questions are required.

Voluntary Critical Illness


Critical illness insurance, available through Reliance Standard, is designed to help you offset the financial
effects of a catastrophic illness with a lump sum benefit if you or a loved one are diagnosed with a covered
critical illness. The critical illness benefit is based on the amount of coverage in effect on the date of diagnosis
of a critical illness or the date treatment is received according to the terms and provisions of the policy.

During benefits enrollment, you can elect this coverage without questions regarding your health.

z Employee: $5,000, $10,00, $20,000 or $30,000


z Spouse/domestic partner: $5,000 or $10,000, not to exceed 100% of employee’s election
z Child(ren): up to 50% of the associate’s election
z Coverage is portable which means you can take this plan with you if you no longer work for the
