Dental
Atrium Hospitality offers a voluntary dental plan through now with United Healthcare!. These plans are
designed to help you get the dental care you need and help lower your costs. With these benefits you receive
a wide range of covered services—both in and out of network.

For services provided by an out-of-network dentist, UHC Dental will reimburse according to the Maximum Reimbursable Charge. The Maximum
Reimbursable Charge is calculated at the 90th percentile of all provider charges in the geographic area. The dentist may bill up to their usual fees.

Atrium Hospitality offers a voluntary vision plan through EyeMed. It helps pay the cost of periodic vision
examination and necessary lenses and frames, if prescribed. The plan covers services from any licensed
provider but benefits are paid at a higher level when you use an in-network provider.

Life and Accidental Death and Dismemberment
(AD&D)
ATRIUM’S LIFE/AD&D PLAN IS ADMINISTERED BY CIGNA/NEW YORK LIFE

z Atrium-paid Basic Life and AD&D at multiple of your base salary.
z Voluntary Life/AD&D—Choice of $10,000 increments to a maximum of $500,000.
z Evidence of Insurability (EOI) is required for elections over $150,000 during initial enrollment.
z Evidence of Insurability will be required for all amounts if electing coverage outside your initial
enrollment.
One-time opportunity for January 1, 2024. You can enroll up to the guarantee issue limit without EOI.
Dependents
z You can purchase Voluntary Dependent Life/AD&D in $5,000 increments up to $100,000 (or 50% of your
voluntary life election) for your spouse and increments of $1,000 to a maximum of $10,000 for your
eligible dependent child. These cannot be enrolled in unless the associate enrolls in Voluntary Life.
z Guaranteed issue for Spouse Life is $25,000. All amounts for Child Life are guaranteed issue.

Short Term and Long Term Disability
ATRIUM’S STD/LTD PLANS ARE ADMINISTERED BY CIGNA/NEW YORK LIFE
In case of an accident or illness, this benefit pays 60% of your base salary to a set maximum. If you are
covered under either one of these plans and believe you have a non-work related disability that prevents you
from working, contact your Human Resource Representative to start the process of filing a disability claim.
You may enroll into STD and LTD when you first become eligible without submitting a Statement of Health for
Medical Evidence of Insurability (MEOI) to Cigna/New York Life. If you decide not to enroll during your initial
enrollment, you will be required to submit the MEOI to Cigna/New York Life.
