Assessment Form

This assessment form evaluates an intern's performance in various areas over the course of their internship. It is divided into several sections that rate the intern on a scale of 1 to 5 in areas such as ability to learn, communication skills, problem solving, work habits, and character. The rater is asked to provide comments for each section and indicate at the end of the form whether they would like to work with and consider this intern again, as well as an overall performance rating of unsatisfactory, average, or outstanding.
