I. Objectives: By the end of the lesson, students will be able to:

1. Understand the importance of filling up forms correctly like bio-data.


2. Identify the necessary information required in a bio-data form.
3. Demonstrate the proper way of filling up a bio-data form accurately.
4. Apply their understanding of filling up forms by completing a mock bio-
data form.

III. Materials:

1. Printed copies of a sample bio-data form
