 Scaffolding collapse.  The ladder should be fixed for safe access. 1:4 ratio or 70 degrees.

 Electrocution hazard  A 12 mm thickness Lifeline should be installed for FBH anchoring.

 A proper platform should be provided for working at height. Ensure
