AFFIDAVIT OF EXPLANATION

I, PCpl Arche S Beljano, of legal age, bonafide member of Philippine National Police
presently assigned at Aglipay Police Station, Aglipay, Quirino after having been duly sworn to
an oath in accordance with law do hereby depose and say;

That I am the same person executing this affidavit of explanation:

That a memorandum received by this office from OIC, POMU directing POMIS encoders
of Police Station to explain in writing (under oath) why no disciplinary action should be taken
against them for late uploading of spot/incident report with-in three-hour period;

That based on the records found in the POMIS account of the undersigned said
attachment was uploaded at about 10:34 PM of the same date (March 06, 2024) which is late
for 4 hours and 4 minutes. That on same day, at about 4:00 PM until 8:00 PM, the undersigned
tried to upload the attachment but due to unstable internet connectivity in our office, the
undersigned did not meet the prescribed time.

That after the undersigned couldn’t upload the said attachment, I participated during our
scheduled checkpoint at around 9:00 PM- 11:00 PM and at around 10:00 PM my colleague
called me and told that our internet connectivity was already restored prompted me to proceed
to our office and uploaded the said file attachment.

That the undersigned has no intention to violate or defy any existing laws, rules and
regulation governing the PNP in the system during the process of encoding.

That I am executing this Affidavit to test the truthfulness of the forgoing facts.

AFFIDAVIT OF EXPLANATION

That I am the same person executing this affidavit of explanation;

That a memorandum received by this office from OIC, POMU directing POMIS encoders
of Police Station to explain in writing (under oath) why no disciplinary action should be taken
against them for late uploading of spot/incident report with-in three-hours period;

That based on the POMIS generated data for the period of April 1-30,2024, one (1)
incident/ spot report of Aglipay PS dated April 23,2024 was uploaded more than the prescribed
period of three- hours;

That based on the records found in the POMIS account of the undersigned, said incident
report was uploaded at 09:34 AM of the succeeding date (April 24, 2024), after the incident has
been reported at 8:00 PM of April 23,2024 which is 10 hours and 34 minutes late within the
three-hours prescribed period. That on that day, when the incident/spot report should be
uploaded at 8:00 PM until 11:00 PM, the undersigned attempted numerous times to upload the
said incident report but due unknown factors, the system was not responding and that was the
reason the undersigned didn’t meet the prescribed period;

That after the undersigned couldn’t upload the said incident/ spot report, I informed the
higher headquarters as well as my Chief of Police about my concern and I was advised by
PMSgt Leo Jose V Wigan Jr. to repeatedly do the uploading until it will be uploaded, so I
exerted effort to upload the same until on the succeeding day, April 24,2024 at 7;30AM, I was
asked again by my COP if I already uploaded the incident but still I can’t access/gain connection
to the POMIS account, until I took a chance to gain accesss at 9:30 AM and that was the only
time the system has responded and uploaded the same;

That the undersigned has no intention to violate or defy any existing laws, rules and
regulation governing the PNP in the system during the process of encoding;

Attached in this affidavit are the real time action photos that the undersigned is
attempting to access the system;

That I am executing this Affidavit to test the truthfulness of the forgoing facts;

presently assigned at Aglipay Police Station, Aglipay, Quirino as the Chief of Police after having
been duly sworn to an oath in accordance with law do hereby depose and say;

That I am the same person executing this affidavit of explanation:

That a memorandum received by this office from OIC, POMU directing POMIS encoder
and the Chief of Police of this Station to explain in writing (under oath) why no disciplinary action
should be taken against us for late uploading of spot/incident report within three-hours
prescribed period;

That based on the POMIS generated data for the period of April 1-30,2024, one (1)
incident/ spot report of Aglipay PS dated April 23,2024 was uploaded more than the prescribed
period of three- hours;

That based on the records found in the POMIS account of PCpl Arche S Beljano, our
POMIS encoder, said incident report was uploaded at about 09:34 AM of the succeeding date
(April 24, 2024), after the incident has been reported at 8:00 PM of April 23,2024 which was 10
hours and 34 minutes late within the three-hours prescribed period, wherein it should be
uploaded at 8:00 PM until 11:00 PM of that day, and I was informed by the former that he
attempted several times to upload the said incident report but accordingly, due to unknown
factors, the system was not responding and that was the reason for the encoder that he didn’t
meet the prescribed period;

That after he couldn’t upload the said incident, PCpl Beljano informed me about the
matter and I directed him to inform the QPPO about our concern and was able to talked with
PMSgt Leo Jose V Wigan Jr., wherein he was advised by the latter to repeatedly do the
uploading until it will be uploaded, wherein the former exerted effort to upload the same, and on
the succeeding day April 24,2024 at 7;30AM, I asked PCpl Beljano if he was done in uploading
but accordingly, he can’t still gain access and he averred that he continues doing the uploading
until he took access on the system and uploaded it at 9:30 AM;

That the undersigned has no intention to violate or defy any directives, existing laws,
rules and regulations governing in the system and process of encoding in the POMIS;

That I am executing this Affidavit to test the truthfulness of the forgoing facts.
